Contractor Monitoring. 1. Contractor shall conduct, at least annually, a utilization review of DMC providers to assure covered services are being appropriately rendered. The annual review shall include an on-site visit of the service provider. Contractor Monitoring. Monitor Contractor’s work and assure that the Contractor is conducting inspections, preparing reports, and monitoring all Stormwater pollution prevention measures associated with the project. For each project that requires the use of the National Pollutant Discharge Elimination System (NPDES) General Permit, provide at least one staff person who has successfully completed the "Florida Stormwater, Erosion, and Sedimentation Control Training and Certification Program for Inspectors and Contractors.” The Consultant’s inspector will be familiar with the requirements set forth in the FEDERAL REGISTER, Vol. 57, No. 187, Friday, September 5, 1992, pages 4412 to 4435 "Final NPDES General Permits for Stormwater Discharges from Construction Sites" and guidelines developed by the City. Problem Resolution Analyze problems that arise on a project and proposals submitted by the Contractor, prepare and submit recommendations to the Construction Manager, and process the necessary paperwork.
Contractor Monitoring. The Contractor agrees to comply with monitoring by the State of all programmatic and financial activity in relation to the approved project. Post-award monitoring may be conducted to determine the Contractor’s progress towards implementing the planned award activities, review compliance with relevant laws and regulations, and provide technical assistance as needed. The Contractor assumes responsibility for ensuring the relevant copies of all records and correspondence are maintained and are accurate and complete. If Contractor awards funds to sub-Contractors, they assume responsibility for all monitoring and compliance of all programmatic and financial activity of said sub-Contractor.
Contractor Monitoring. Contractor’s Responsibility Contractor shall establish a protocol to monitor the program funded by this Agreement. Monitoring shall include, but is not limited to, a review of participant eligibility, data entry participant time and attendance, verification of documents and reports prepared and/or submitted to LVWDB by Contractor, participant progress reports, invoices and fiscal records. The Self-Monitoring required by this Agreement shall be in addition to the monitoring conducted by the LVWDB. Monitoring Schedule Contractor’s monitoring protocol shall be conducted in accordance with a schedule approved by the LVWDB. Contractor shall submit their monitoring schedule to the LVWDB by July 31 of each program year that this Agreement is in effect. Contractor’s Monitoring Report and Corrective Action Within five (5) business days following the completion of the self-monitoring protocol Contractor shall submit a copy of their monitoring report which shall include any findings identified and Contractor’s proposed corrective action to the LVWDB. The LVWDB shall inform Contractor in writing regarding acceptance of the proposed corrective action or shall provide Contractor with additional instructions on resolving any of the findings. Failure to take the requisite corrective action as a result of any monitoring findings may result in a determination that Contractor has failed to meet a performance element as described in Article 7, a recommendation not to renew this Agreement or non-payment of a cost related to a fiscal finding. If the Commonwealth and/or federal oversight entity does not agree with the resolution of a questioned or disallowed cost as a result of a monitoring or audit finding, Contractor shall comply with the Commonwealth or federal determination and in the event of a questioned or disallowed cost Contractor shall be liable to LVWDB for repayment of the cost which has been questioned or disallowed. Coordination of Monitoring with Annual DLI Visit Self-monitoring shall include a one hundred percent (100%) file review each year this contract is in effect. The file review shall be coordinated to occur thirty (30) days prior to the notice of the DLI’s annual monitoring visit. The file review shall incorporate the current program year and the immediately preceding program year.
