Contract Discrepancy Report. Verbal notification of a complaint will be made to the County’s Contract Project Monitor as soon as a problem is identified. The problem shall be resolved within a time period mutually agreed upon by the County and the Contractor. The County’s Contract Project Monitor will issue a formal Contract Discrepancy Report (Statement of Work Exhibits, Exhibit 1 – Contract Discrepancy Report). Upon receipt of this document, the Contractor is required to respond in writing to the County Contract Project Monitor within five (5) business days, acknowledging the reported discrepancies or presenting contrary evidence. A plan for correction of all deficiencies identified in the Contract Discrepancy Report shall be submitted to the County Contract Project Monitor within ten (10) business days.

Contract Discrepancy Report. (CDR) Performance of a required service is considered acceptable when the number of discrepancies found during Contract monitoring procedures do not exceed the number of discrepancies allowed by the AQL. When the performance is unacceptable, the CONTRACTOR shall be required to respond within five (5) business days, to a Contract Discrepancy Report (CDR). The CDR will require the CONTRACTOR to explain, in writing, the reasons for such unacceptable performance, how performance will be returned to an acceptable level, and how recurrence of the problem will be prevented. CCA will evaluate the CONTRACTOR's explanation and determine if any financial penalties will be assessed. The CDR is at the end of this exhibit as Attachment II to Technical Exhibit 6.1.

Contract Discrepancy Report. (CDR) - A report prepared by the Quality Assurance Evaluator to inform the CONTRACTOR(s) of faulty service. The CDR requires a response from the CONTRACTOR(s) explaining the problem and outlining the remedial action being taken to resolve the problem within five (5) business days after receipt of CDR.
