Application format Each application must include all contents of the application package, in the order indicated, and conform to the following formatting specifications: • Use 8.5” x 11” letter-size pages (one side only) with 1” margins (top, bottom, and sides). Application Kit SAMHSA application kits include the following documents: • PHS 5161-1 (revised July 2000) – Includes the face page (SF 424 v2), budget forms, assurances, certification, and checklist. You must use the PHS 5161-1. Applications that are not submitted on the required application form will be screened out and will not be reviewed. • Request for Applications (RFA) – Provides a description of the program, specific information about the availability of funds and instructions for completing the grant application.
