Community Service and Self-Sufficiency. Each non-exempt adult resident of the Tenant’s household who is eighteen (18) years or older shall spend eight (8) hours a month performing documented community service, engaging in an economic self-sufficiency program, or a combination of both. The HACB will not renew this lease if the family has violated the requirements to perform community service or economic self-sufficiency activities.

Community Service and Self-Sufficiency. The Resident and adult members of his or her household shall perform eight (8) hours of community service (as such term is defined in HUD regulations) or participate in an economic self-sufficiency program (as such term is defined in HUD regulations) for eight (8) hours each month unless otherwise exempt under HUD regulations. The Landlord shall make an annual determination regarding the compliance of the Resident and members of the household with the community service and economic self- sufficiency requirements no later than thirty (30) days prior to the expiration of this Lease. If the Landlord determines that the Resident or a member of Resident’s household is non-compliant, the Landlord shall notify the Resident of the determination. The Lease shall not be renewed at the end of the current term unless the Resident or the member of the Resident’s household who is non-compliant enters into a written agreement with the Landlord to make up the missed hours, or the Resident’s household provides written assurance satisfactory to the Landlord that the non- compliant member no longer resides in the Unit. The Resident may request a grievance procedure on the Landlord’s determination of non- compliance.

  • Community Engagement The HSP will engage the community of diverse persons and entities in the area where it provides health services when setting priorities for the delivery of health services and when developing plans for submission to the LHIN including but not limited to CAPS and integration proposals. As part of its community engagement activities, the HSPs will have in place and utilize effective mechanisms for engaging families, caregivers, clients, residents, patients and other individuals who use the services of the HSP, to help inform the HSP plans, including the HSP’s contribution to the establishment and implementation by the LHIN of geographic sub-regions in its local health system.

  • Community Service Leave Community service leave is provided for in the NES.

  • Citizen Volunteer or Community Service Leave Leave without pay may be granted for community volunteerism or service.

  • Community Involvement The Grantee will facilitate and convene a Community Task Force as one means of developing collaboration among the Grantee, affected residents, and the broader community. The Grantee also will provide information to keep the Community Task Force fully apprised of the planning and implementation of revitalization efforts. The Community Task Force shall be comprised of affected public housing residents, local government officials, service providers, community groups, and others. The Community Task Force will provide advice, counsel and recommendations to the Grantee on all aspects of the HOPE VI development process, including shaping the goals and outcome of the Community and Supportive Services Plan. Community Task Force participants also will disseminate information throughout the community about the Grantee's revitalization efforts. The Grantee's responsibilities with regard to the Community Task Force include:

  • Community Based Adult Intensive Service (AIS) and Child and Family Intensive Treatment (CFIT) – AIS/CFIT programs offer services primarily based in the home and community for qualifying adults and children with moderate- to-severe mental health conditions. These programs consist at a minimum of ongoing emergency/crisis evaluations, psychiatric assessment, medication evaluation and management, case management, psychiatric nursing services, and individual, group, and family therapy. In a Provider’s Office/In Your Home This plan covers individual psychotherapy, group psychotherapy, and family therapy when rendered by: • Psychiatrists; • Licensed Clinical Psychologists; • Licensed Independent Clinical Social Workers; • Advance Practice Registered Nurses (Clinical Nurse Specialists/Nurse Practitioners- Behavioral Health); • Licensed Mental Health Counselors; and • Licensed Marriage and Family Therapists. Psychological Testing This plan covers psychological testing as a behavioral health benefit when rendered by: • neuropsychologists; • psychologists; or • pediatric neurodevelopmental specialists. This plan covers neuropsychological testing as described in the Tests, Labs and Imaging section.
