Common use of Client Data Clause in Contracts

Client Data. The Subrecipient agrees to maintain client data demonstrating client eligibility for services provided. Such data shall include, but not be limited to, each household member’s name, address, ethnicity, race, gender, age, head of household, income level, disability, homeless status, or other basis for determining eligibility, and a description of the service provided. Documentation of homeless status must be documented by written observation of outreach worker; or written referral by another housing or service provider; or certification by the individual or head of household seeking assistance stating that (s)he was living on the streets or in shelter. For victim service providers, an oral statement by the individual or head of household seeking assistance which states that they are fleeing; they have no subsequent residence; and they lack resources. This statement must be documented by self-certification or a certification by the intake worker. Confidentiality of records pertaining to the provision of family violence prevention or treatment services with assistance under this program as set forth in 42 U.S.C. 11375 (c) (5), must be maintained. All data will be entered into HMIS.
