Child and Family Team Meetings. A family-centered, strength-based, collaborative process to develop a plan of care in the best interest and protection for the CLIENT and familial support. CONTRACTOR shall participate in Child Family Team meetings at the request of the CLIENT/FAMILY or ADMINISTRATOR.

Child and Family Team Meetings. Child and Family Teams include family members, caregivers, “natural supports” and community and agency members that come together to create, implement, and update a plan with the child/youth, and family on behalf of the child. The plan builds on the strengths of the child, youth, and family and addresses the child’s needs, and desires. The plan for the child/youth is developed with the Child and Family team, within the context of a Child and Family Team Meeting (CFTM). Per regulations, the CANS shall be completed during the Child and Family Team Meeting. Certain individuals in the child’s life are mandated to be included in the Child and Family Team, and the CFTM’s, but the youth should also identify members that they would like to be on their team. The mandated members include:
Child and Family Team Meetings. The INTAKE SPECIALIST and the CLINICIAN may be invited to be part of the Child and Family Team (CFT) to support children and youth at the CENTER. They shall participate in the CFT meetings to share whatever knowledge or information they may have that may be useful in the planning, delivery, and management of safety, wellbeing and permanency of the children and youth.

In the event of illness or Board-approved absence, the Superintendent's designee shall attend such meetings.
