Chapter Summary. The majority of the above reports were done around 2000. Judging by the reports described above, evidence shows that they are basically focusing on the causal factors of lifeboat accidents and subsequently the improvement measures from lessons learned. These reports were strongly regional. Most of them as well as accident investigations were done by UK. Consequently the deviations with other countries are yet unknown. In the various reports, some basic accident classifications were done and causal factors were analyzed in details. Hence, some information in the previous studies is able to be applied to the thesis. The present lifeboat accident level is still unknown due to limited sources of marine casualties and incidents data. Although appeals of reducing lifeboat accidents stopped showing up after the MSC.1/Circ.1206 in 2006, it is still able to see lifeboat accidents involving casualties and injuries after that with initial causes of technical failure and human error from GISIS. There is a 3 years gap between the last investigations on lifeboat accidents and the current status with lifeboat accidents kept happening especially 3 GISIS reported accidents in 2007. Particularly, seeing through the report of Xx. X. X. Xxxx, a lot of uncertainties will be found which sourced from the questionnaire and hypotheses. In the risk assessment of an accident, data and information should be as complete as possible to perform an accurate analysis. As we know, owing to the inherent imperfect accident analysis, data and information will be uncertain more or less. Modeling assessments by applying probabilistic measures would be beneficial to get more reliable results. In the view of this, performing an intuitive visual format such as an intuitive visual format such as Influence Diagram and succeeding a Bayesian Network which is more and more popular in analyzing marine accidents could be significant and state-of-the-art. CHAPTER3 ACCIDENT INVESTIGATION
Chapter Summary. There are a number of pathways that prisoners can follow to release and their return to the community. These pathways can be broadly divided into those that lead to release from court and those that lead to release from prison. For prisoners with a severe mental illness, there also needs to be consideration of pathways that are taken through prison mental health services. These pathways are not mutually exclusive and the way that a prisoner moves through the Criminal Justice System and how they come to release impacts the care that prison mental health services can provide. As has been demonstrated, released prisoners have poor outcomes in a number of areas. For prisoners with a severe mental illness, the transition from prison to the community is difficult period and there are high levels of drop out from services, with many prisoners making no contact with community mental health services after release. Alongside this lack of contact with community services, the risk of mortality and other negative health outcomes is elevated and levels of reoffending are high. There are a number of reasons that outcomes are so poor in this period with problems related to health service provision, the prison system and the situation that prisoners find themselves in on release. Some of these problems are well embedded into prison and health systems and would require major changes to their policy, structure and culture, however, it possible that modest interventions could work to mitigate some of their negative effects. Other problems lend themselves to simple solutions and could be resolved through innovative approaches to care. The following Chapter will outline several strategies to improving care in the transition from prison to the community and will describe the background and model of the intervention that this thesis seeks to evaluate.
Chapter Summary. The Critical Time Intervention will be evaluated in this thesis and it aims to improve outcomes during and after the transition to the community for prisoners with a mental health problem. It provides additional support both before and after release from prison and tries to ensure that discharge planning takes place and that contact with community mental health services is facilitated. The CTI has a growing evidence base in the USA in a range of settings and a pilot study has confirmed it is feasible in prisons in England and Wales. There are other interventions which have been used in this transition, but published studies of their efficacy are low in number, are of a poor quality and have varied outcomes. In addition, there are examples of other approaches in the transition from other settings to the community that have varying effect. The CTI has been used in this thesis because of its evidence base, its appropriateness to the context and its mid-level intensity and cost. This thesis will allow an evaluation of its effectiveness in the prison setting in England, using a larger sample and more rigorous method than was possible in the pilot. Chapter 5. Chapter Summary. In this chapter data has been analyzed on the influence of strategic alliance between co- operative bank and Safaricom limited enhance performance of the two organizations. The chapter also includes the discussion of the study which has been linked to the theory and linked to other studies. The chapter indicates that alliance was a hybrid alliance as it focused on obtaining increased sources of revenue as well as obtaining knowledge that could be perhaps utilized during alliances with other organizations. For the alliance to be successful, there is need to focus attention and resources on building corporation wide alliance capabilities. Chapter Summary. The methodology chapter presents an overview and an argument for the research design, sample, interview protocol and process, data analysis and ethical issues adopted in the current research. The semi-structured interview was identified as an efficient tool to use and explore the teachers’ attitudes towards inclusive education and identification their roles in the implementation of inclusive education in mainstream schools in Northern Kazakhstan. The application of the purposeful sampling allowed the researcher to recruit seven participants in the current study who provided the necessary data about their experience and knowledge about inclusive education and defined their roles as agents of change in implementing inclusive education in mainstream schools.
