Certification of Leave. To be eligible for military leave or emergency call up pay, the employee must provide the Employer with a certificate from the commanding officer of the employee’s unit that the leave taken was for either such purpose.
Certification of Leave. When an employee requests leave, the employee shall provide certification, in accordance with FMLA regulations, of the need for leave. Fitness for Duty In accordance with administrative regulations, when an employee that takes FMLA due to the employee’s own serious health condition, the employee shall provide, before resuming work, a fitness-for duty certification from the health care provider.
Certification of Leave. The College will require certification for your/your family member’s serious health condition, for the qualifying exigency for military family leave, or for the serious injury/illness of a covered servicemember, as follows:  Employee’s/Family Member’s Serious Health Condition. You must respond to such a request within 15 days of the request or provide a reasonable explanation for the delay. Failure to provide certification may result in a denial of leave or continuation of leave. The certification form to be used is available from the Human Resources Office. The Human Resources Office may directly contact your/your family member’s health care provider for verification or clarification purposes, but will first give you the opportunity to resolve any deficiencies in the medical certification and will obtain your/your family member’s written permission for clarification of individually identifiable health information. The College has the right to ask for a second opinion if it has reason to doubt the certification provided. The second opinion will be obtained from a health care provider of the College’s choice at the College’s expense. If necessary to resolve a conflict between the original certification and the second opinion, the College will require the opinion of a third health care provider of its choice at its expense. This third opinion will be considered final. You will be provisionally entitled to FMLA leave and benefits pending the second and/or third opinion. You may be denied FMLA leave if you refuse to release relevant medical records to the health care provider designated to provide a second or third opinion.

  • Verification of Leave Upon return from leave the employee will provide the necessary claim form for verification of absence to his/her principal/supervisor within five (5) workdays in accordance with §1012.61, Florida Statutes.

  • Termination of Leave Interruption of pregnancy will terminate the leave. Human Resources may require in such cases forty-five (45) days notice to return.

  • Duration of Leave An eligible female employee may apply for pregnancy leave, to commence after the 22nd week of pregnancy for a duration of up to 17 weeks. The pregnancy leave of an employee who is not entitled to take parental leave ends on the later of the day that is seventeen weeks after the pregnancy leave began or the day that is six weeks after the birth, still-birth or miscarriage.

  • Extension of Leave 5 If an extension of the leave is required, a request for the extension must be 6 submitted in writing at least five (5) days in advance of the leave expiration 7 or as soon as practical. Consideration of an extension will be based on the 8 same criteria as the original request. Failure to return to work at the 9 expiration of the leave may result in termination.

  • Certification of Coverage Engineer shall furnish County with a certification of coverage issued by the insurer. Engineer shall not cause any insurance to be canceled nor permit any insurance to lapse. In addition to any other notification requires set forth hereunder, Engineer shall also notify County, within twenty-four (24) hours of receipt, of any notices of expiration, cancellation, non-renewal, or material change in coverage it receives from its insurer.
