Certification Fee Clause Examples
Certification Fee. Providers shall pay an “Certification Fee” before ADT shall undertake an evaluation. The Certification Fee varies based upon organization annual gross revenue:
8.1.1. Small Organization ($2,500). A small company that has annual revenue of less than $5,000,000 (USD).
8.1.2. Medium Organization ($5,000). A company with annual gross revenue greater than $5 million (USD) but less than $100 million (USD).
8.1.3. Large Organization ($7,500). A large company with greater than $100 million (USD) in annual revenue.
8.1.4. An additional $50 fee shall be paid by Provider if wiring funds to offset bank fees.
Certification Fee. The Hospital will pay the certification fee for those employees who become certified OR Techs after January, 1984.
Certification Fee. The Company will pay B Lab an annual certification fee (the “Certification Fee”) to be determined based upon the Company’s revenue, which shall be disclosed to B Lab annually, and based upon the structure set forth on B Lab’s or, if applicable, the Global Partner’s (as defined below) website, which may be revised from time to time.

Certification Fee. Practitioner must pay ELSO a Certification Fee in an amount indicated by ELSO. The Exam Fee is non-refundable and non-transferable. For the avoidance of doubt, payment of the Exam Fee does not entitle Practitioner to any rights or privileges under this Agreement.
Certification Fee. 1. All Audit activities including application, initial certification and surveillance audits shall be charged as per the ‘FEE STRUCTURE’ attached.
2. If the Audit team decides that nonconformity, found during Audit, should be verified through follow-up visit, verification Audit shall be performed.
3. All boarding, lodging & traveling expenses shall be charged to Client.
Certification Fee. (a) In consideration of the rights granted to Instructor herein, Instructor shall pay FR a nonrefundable monthly certification fee of $25.00 a month for each month following the first anniversary of the Agreement Date (the “Certification Fee”); certification fees for the Initial Term shall be included in the cost for the Training Course. The first payment of the Certification Fee is due and payable on the date that is the first anniversary of the Agreement Date and each subsequent payment will be made 30 days after the date of the immediately preceding payment of the Certification Fee. Except that the Certification Fee shall not apply to an instructor whose cost for the Training Course was, or will be, paid by Fluid Running.
(b) Notwithstanding 4(a) an instructor certified on or before November 1, 2016 and whose Training Course fee was not paid by Fluid Running shall not be subject to the Certification Fee but shall pay an annual Certification Renewal Fee (the “Renewal Fee”) equal to $195. The Renewal Fee shall be paid on or before January 15th of each year.

8.1.1. Startup Provider ($2,000). A startup provider is a participant that is less than 4 years old.
8.1.2. Regular Provider ($4,000). A regular provider is a participant that is not a start-up organization or a large provider (below).
8.1.3. Large Provider ($6,000). A large provider is a participant with annual sales greater than $100 million.
8.1.4. An additional $50 fee shall be paid by Provider if wiring funds to offset bank fees.
