Catastrophic Leave Committee. The Catastrophic Leave Committee shall administer the Catastrophic Leave Program. The Committee will be established on an annual basis at the conclusion of each school year for the following year and composed of two (2) representatives each of the California School Employees Association and Turlock Classified-American Federation of Teachers as appointed by the presidents of the bargaining units; two (2) Confidential members appointed by the Superintendent or designee; Superintendent’s designee/Director of Human Resources; one non-voting classified representative of the Fiscal Services Department; and one non-voting classified representative of the Human Resources Office. The Catastrophic Leave Committee will determine the eligibility of such leave requests on a case by case basis.
Catastrophic Leave Committee. An Association-District Catastrophic Leave Committee comprised of three representatives from the Association, appointed by the Association President, and a non- voting member from the District, appointed by the Superintendent, shall administer the provisions of this article. The duties and obligations of this committee shall include the following:
Catastrophic Leave Committee. A Catastrophic Leave Committee (CLC) shall be established consisting of two (2) bargaining unit members appointed by the Association and two (2) District representatives appointed by the Superintendent or his/her designees. The CLC shall have the following authority and guidelines:
Catastrophic Leave Committee. The Catastrophic Leave Committee is comprised of three representatives from CSEA as voted by the CSEA unit. Additionally, the committee will include a non-voting CSEA representative from the payroll department to be determined by the Business Department management to act as an advisor as needed. Each year CSEA will submit a memo to the Santa Xxxx County Office of Education Human Resources Director with the names of the Committee Members.
Catastrophic Leave Committee. 12:11.1 The committee will be comprised of two (2) NFT representatives and one (1) District administrator who will determine and certify that the certificated employee is eligible for catastrophic leave because he/she is unable to work due to the severity of that personal, family or domestic partner/companion catastrophic illness, and only after adequate proof of illness has been provided in accordance with Education Code Section 44043.5.
Catastrophic Leave Committee. The approval committee shall consist of a representative of the District, CSEA Chapter 420 President or designee, and a CSEA Chapter 420 member appointed by the CSEA Executive Board.

Catastrophic Leave Committee. A. The Catastrophic Leave Committee shall consist of three (3) members, two (2) appointed by the President of the Association (CSEA #105), and one (1) appointed by the Superintendent.
Catastrophic Leave Committee. 6.9.1.1 A Catastrophic Leave Committee shall be created and will consist of two District representatives and three representatives appointed by SMEEA. The Committee shall evaluate all requests for catastrophic leave and may request additional information. The Committee shall have the authority to grant, partially grant, or reject any requests. All decisions to grant catastrophic leave shall require the agreement of a majority of the Committee members.
Catastrophic Leave Committee. A Catastrophic Leave bank committee will be composed of the Vice President of Human Resources and Employee Relations and two appointees of the Federation. Days contributed to the bank by unit members shall not be withdrawn for other than catastrophic leave. The Catastrophic Leave bank committee will accept or reject use of days from the bank. Unit members applying for Catastrophic Leave days must submit a letter of request along with a signed and dated doctor’s statement verifying the probable duration of the illness or injury.
