CASE NARRATIVES. 7.1.1 Accurately maintain and update case narratives as required by ADMINISTRATOR. All entries by CONTRACTOR are to be entered into the case record in a format approved by ADMINISTRATOR. Case narratives shall include, but are not limited to, the following items: Date WTW case is assigned; Current status of the case, including assessment of service needs and barriers to participation, actions taken, and status of referrals; Dates, including but not limited to; appointments, case closures or transfers; pending and/or incomplete actions; and the type of contact for all communication, including required weekly and/or monthly contact(s); Assigned WTW Activities; Required weekly participation hours; Use of mandatory CalWORKs 2.0 tools; Case Manager name and caseload number after each entry; and/or Any additional information as required by ADMINISTRATOR.

All entries by 6 CONTRACTOR are to be signed, dated, legible, and in a format approved by ADMINISTRATOR. 7 Case narratives shall include, but are not limited to, the following: 8 Date referral is received, assessment of service needs, actions taken, and 9 status of referrals. 10 Overall plan for PROGRAM PARTICIPANT, outcomes and follow-up 11 dates arranged during contact. 12 Weekly participation hours. 13 Complete and accurate descriptions of the case activity. 14 Issues related to the PROGRAM PARTICIPANT’s progress toward the 15 established TILP and/or Needs and Services Plan. 16 The closing narrative shall include date and reason for the termination, 17 goals completed, and referrals to resources for continued support.
