Case Management Services. Services that develop case plans for delivering community services to PATH eligible Consumers. The case plans should be developed in partnership with Consumers who receive PATH services to coordinate the assessment, treatment, housing and/or care of Consumers, tailored to Consumer needs and preferences. Case Managers assist the Consumer in accessing needed services, coordinate the delivery of services in accordance with the case plan, and follow-up and monitor progress. Activities may include financial planning, access to entitlement assistance, representative payee services, etc. Case Management Services include:

Case Management Services. Services that assist PROVIDER in designing and implementing strategies for obtaining services and support that are goal oriented and individualized and that assist consumers with access to needed health services, financial assistance, housing, employment, education, social services and other services. PROVIDER must comply with MSHN Case Management Policy and in accordance with the MSHN-SUDSP Manual.
Case Management Services. Case Management services will be provided to the persons who are homeless / chronically homeless who utilize this low-barrier shelter and service center. The case managers will aid in triaging participants to services that best meet their needs, and then link them to those area services. The goal of low-barrier services is to provide progressive engagement to chronically homeless who are challenging to serve and reluctant to seek assistance due to mental health challenges, substance use, and chronic health conditions. Once linked to existing services within the County, participants will be on a path that leads towards permanent supportive housing, alternative stable housing, and stability. Case managers may also help divert people from shelter through family reunification and other means. Through linkages and diversion Case Managers will work to decrease length of stay at the OC Courtyard and increase stability. This project will align with the Orange County Continuum of Care (CoC) and coordinated entry system. Staff will receive Orange County CoC / CES / or other training offered in best practices and local CoC process.

Case Management Services. An ongoing process to assist eligible clients gain access to and effectively use necessary health and related social services. Case management is used to either involve eligible clients in substance use disorder treatment or to support them as they move through stages of substance use disorder treatment within or between separate treatment agencies. Services are delivered to enrollees with substance use disorders who need assistance in obtaining necessary medical, social, educational, vocational and other services
Case Management Services. Case Management services shall be provided to the program 22 participants who utilize this low barrier shelter and access available resources offered by the
Case Management Services. We may provide, when appropriate for the individual Participant, certain case management services, which are designed to provide a proactive, systematic process of coordination of health care services, including the evaluation of inpatient, outpatient and ancillary services, member education, the review of the short term outpatient care needs and where appropriate, coordination and facilitation of discharge planning needs. These services address the unmet health care needs of Participants who are not eligible for a disease management program under the Plan but are at significant risk for declining health status and high medical expenses. We also provide an Alternative Benefit Program (ABP) of benefit coverage for health care services. This ABP program is designed by us for the diagnosis and/or treatment of a particular Participant’s illness or injury with appropriate and cost effective health care services and supplies alternatives that would otherwise not be covered by the Plan. The Plan will pay for and cover as Plan benefits the health care services and supplies contained in the ABP program. You consent to our use and administration of the ABP program and delegate to us the discretion and authority to develop and revise ABPs. We will work with Participants who satisfy the criteria for participation in case management services to develop a program of benefit coverage with appropriate and cost-effective heath care services and supplies for the diagnosis and/or treatment of the Participant’s condition. If the Participants and health care provider are not willing to participate in the process, we will not provide these services.
