Case Conferences. Any teacher involved in the "Mainstreamed" Program may participate in any case conference relative to their student's program. Teachers shall be released during case conferences held during the school day. Such release shall not infringe upon regular preparation time. Teachers shall be compensated at their regular hourly rate for participation in case conferences held outside the regular school day.

Case Conferences. As needed and when the conference involves a current or former guest of the program, Grantee shall participate in individual case conferences and team coordination meetings with HSH-approved programs, as needed, to coordinate and collaborate regarding participants’ progress.
Case Conferences. All teachers requesting compensation for hours spent in Case Conference Meetings, Annual Case Review Meetings, Strength Based Assessments and Student Support Teams beyond the maximum workday shall document the hours worked on a form provided by the administration. Each hour worked shall equal one hour of personal leave. When these hours accumulate to one (1) teaching day as outlined in the contract, one (1) Personal Day will be added to their accumulated Personal Leave, not to exceed five (5) days per school year. If the Personal Day is not used during the current school year, and the total is over five (5) Personal Days for that year, it will roll over into the teacher’s accumulated sick days. If one (1) teaching day as outlined in the contract is not reached in a school year, the hours shall be carried over to the following year.
Case Conferences. The Supplier shall attend and participate in case conferences as required by City of York Council. Case conferences shall take place on an ad-hoc basis for any complex cases, to monitor attendance management cases and shall be conducted for one or more cases as requested by the City of York Council. The Supplier shall: Ensure case conferences focus on recommendations to resolve long-term sickness absence and cases of repeated short term absences where a medical condition may be the cause; Provide verbal and written case reports including a summary of the case, prognosis, likely length of absence, reasonable adjustments required and recommendations and actions required by either the Supplier, City of York Council and/or the City of York Council Personnel; and Provide the City of York Council with details of any recommendations made by the Supplier to the City of York Council Personnel and with which the City of York Council Personnel disagrees. The Supplier shall be advised that attendees at case conferences may include line management, HR, relevant Supplier Personnel (such as the occupational health advisor), City of York Council, health and safety advisor, a trade union representative and/or legal advisor, where the City of York Council Personnel has given prior agreement. The Supplier shall be advised that case conferences shall be delivered by telephone, face-to-face, video, on-site and/or off site as required by the City of York Council. The Supplier shall ensure that ad hoc case conferences take place within 5 working days of request by the City of York Council. The Supplier shall ensure multiple case conference (including collation of referrals) take place within ten (10) working days of request by the Contracting Authorities.
