Care Transitions a. The PHP shall make best efforts to coordinate with the Department to ensure that Members seeking placement in institutional settings are not placed in settings that the Department has identified as in noncompliance with Federal and State guidance on public health infection control and prevention.

Care Transitions. Contractor will utilize existing partnerships and protocols with local hospitals to accept acute care discharge referrals and assist with care transitions from hospital to home through June 30, 2020. This may include referrals from HCS for clients that do not qualify for Medicaid LTSS and require care transitions provided by the Area Agency on Aging. Basic client demographics, enrollment in CLC Care Transitions for COVID-19 Response, and hours of service provided for each client must be tracked in CLC GetCare. Pursuant to 42 CFR §431.301 and §431.302, personal information 41 concerning applicants and recipients may be disclosed for purposes directly connected with 42 the administration of this Contract and the State Medicaid Plan. Provider shall read and 43 comply with all HIPAA policies.

  • Passwords and Employee Access Provider shall secure usernames, passwords, and any other means of gaining access to the Services or to Student Data, at a level suggested by Article 4.3 of NIST 800-63-3. Provider shall only provide access to Student Data to employees or contractors that are performing the Services. Employees with access to Student Data shall have signed confidentiality agreements regarding said Student Data. All employees with access to Student Records shall pass criminal background checks.

  • Community Engagement Integration Activities The SP will support the HSP to engage the community of diverse persons and entities in the area where it provides health services when setting priorities for the delivery of health services and when developing plans for submission to the LHIN including but not limited to CAPS and integration proposals.
