Care Planning. Once enrolled with Providence ElderPlace/PACE, the interdisciplinary team will meet with you to evaluate your needs and goals and talk with you about services. Your individual care plan is developed by you, your primary care team, and your family or other chosen representative. For any service needed, simply make a request to a member of your care team. All services approved by the interdisciplinary team are fully covered.
