Cafeteria Services. Mail / Copy Center ------------------------------------------------------------------------------------------------------------------------------------ Annex A ------- Human Resources: Insurance Coverage and Participation in HR Plans and Programs --------------- ------------------------------------------------------------- The charges for insurance coverage and participation by the Company employees in product, worker safety and environmental programs and in benefit plans maintained by CSI shall be calculated on a quarterly basis as follows: The charges for any insurance coverage, or participation in benefit plans and programs shall be equal to (i) the costs incurred by CSI for such coverage, plans and programs (excluding costs related to benefits administration), multiplied by (ii) a fraction that is equal to (A) the salaries, in the ------------- aggregate, of all participating employees of the Company divided by (B) the ---------- salaries, in the aggregate, of all participating employees of CSI and its affiliates (including the Company), each as of the beginning of the relevant quarterly period, provided, however, that CSI shall have the right to allocate -------- ------- certain costs directly to individual employees of the Company where CSI, in its sole discretion, deems it appropriate. The money that each student brings for lunches will be put into his or her individual account. They will be assigned a PIN number, which they will keep for the rest of their elementary career. The Cafeteria Lead will notify students when their account is getting low. Please send your child's money in a sealed envelope with the teacher's name, room number, your child's name, and the amount enclosed. Have your child give this envelope to the teacher as soon as school begins. FREE/REDUCED PRICE LUNCH and BREAKFAST Available to those families who find it a financial necessity. Application forms must be filled out and turned in on an annual basis. These forms are provided at the beginning of school, or you may obtain one from the office. My School Bucks You can go online to add money or view balances of your child’s lunch account at xxx.xxxxxxxxxxxxx.xxx Your My Lunch Money email address will be your My School Bucks username, and Library Books Students visit the library once a week to receive instruction and to check out books. The student and parent will agree to handle library books carefully, return them promptly, and pay for any loss or damage.  The checkout period is one week.  Overdue notices, with information about the title, author and price of the book are first given to child when the book becomes past due.  A second notice will be mailed to the parent. Please respond quickly and call the school librarian with any questions. Please ask the library staff if you need help in locating books to read with your child.

Once a student reaches this limit, an alternative meal (i.e. cheese/peanut butter sandwich and milk for lunch; cereal with milk for breakfast) will be provided for the student until the student’s charges have been paid. Frequent communication will be made with students/parents if the student has charges that need to be cleared up. Parents may prepay cafeteria service in the administrative office or by logging on at xxx.xxxxxxxx.xxx. Students with outstanding charges at the end of the school year may not be issued final records or a class schedule for the next school year until charges have been paid. *Note- If a student had cafeteria charges prior to qualifying for free and/or reduced lunch for longer than a period of one week, those charges are still owed to the District. **Note- Free and Reduced lunch applications must be completed and turned in EACH YEAR. Parents are strongly encouraged to continually monitor their child’s meal account balance. When a student’s meal account is depleted, the district will notify the parent. The student will be allowed to continue purchasing meals for up to $25 and the district will present the parent with a schedule of repayment for any outstanding account balance. If the district is unable to work out an agreement with the student’s parent on replenishment of the student’s meal account and payment of any outstanding balance, the student will receive an alternate meal.

Cafeteria Services. Landlord shall provide Tenant and its employees with unrestricted access to the cafeteria in the Building and for Tenant’s use of related catering services and vending machines (collectively, the “Cafeteria Services”) during normal operating hours of the cafeteria. Tenant acknowledges that Landlord may close the cafeteria from time to time for meetings and special events. Landlord’s obligations hereunder shall extend only to making such Cafeteria Services available to Tenant and its employees but it is understood that Tenant and its employees will pay for actual foods consumed or used by Tenant and its employees and any use of the cafeteria by Tenant for meetings and special events may be provided to Tenant upon request and in accordance with the fee schedule in EXHIBIT “C” .
