Bobtail a tractor without a trailer shall be for on the basis driving Sectio n MILEAGE RATES Mileage rates are laid out by areas and w i l l be paid for such and they are composite mileage rates to compensate for duties performed in normal operations which include driving, checking equipment and reports, including fueling, hook-up unhooking at the origin destination points. The official state, provincial and/or territorial mileages will used as guide to determine the miles driven. The authority to determine the of miles driven w i l l be the Department of Highways of the various provinces, territories and/or states.

If CONTRACTOR desires CARRIER to obtain either bobtail and deadhead or comprehensive, collision and theft insurance for him rather than obtaining it himself, the premium will be charged back against CONTRACTOR. If this option is elected, Exhibit C shall be attached to this lease which will be a certificate of insurance showing the name of the insurer, the policy number, effective dates, amounts and type of coverage, the cost of the policy which is hereby authorized by CONTRACTOR to be deducted from settlements and the deductible amount for which CONTRACTOR may be liable. Upon request CONTRACTOR will be provided with a copy of the policy.

In addition, the Website will provide CLEC with contact information for the personnel and/or organizations within CenturyLink capable of assisting CLEC with inquiries regarding the ordering, provisioning and billing of Interconnection, UNE and resale services. Included in this information will be the contact information for a person or persons to whom CLEC can escalate issues dealing with the implementation of the Agreement and/or for assistance in resolving disputes arising under the Agreement.
