BLUE CROSS AND BLUE SHIELD. A. The Employer agrees to pay for each full-time regular employee the cost of the Massachusetts individual basic Blue Cross and Blue Shield plans. Eligibility shall become effective on the twentieth day of the month following three months of continuous full-time employment.

BLUE CROSS AND BLUE SHIELD. (a) The Town of Tiverton shall supply all active Officers of the Tiverton Police Department Blue Cross/Blue Shield plan “HEALTHMATE” Coast-to-Coast, as described below. Changes to Health Insurance: - Hospital emergency room coverage will include a $100 co-pay unless the user admitted to the hospital, in which the co-pay shall be waived - Urgent care center coverage will include a $50 co-pay - Utilization co-pays Doctor’s visits will increase from $10.00 to $15.00 and for specialists from $15.00 to $25.00 - Prescription co-pays at $7/$25/$40 - Any bargaining unit member may choose to receive added health insurance benefits from plans available to and provided under this Agreement. The Officer must pay the entire difference in the premium rate between what the Town is paying and the full cost of the coverage in addition to the co-share amount. Health Care Plan administrator to be selected at discretion of Town.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.
