Benefits During Leaves of Absence. Employees on leave of absence of one month or less will continue to accrue all benefits available to them in this collective agreement as though they had worked. Employees on leave of absence of longer than one (1) month in duration who choose to, and if they are eligible to do so, may maintain their benefits by paying the entire premiums themselves. Sick benefits will be maintained but not accrue past the one calendar month until the employee’s return to work and vacation pay will be based on the percentage of gross earnings and not the greater of calculation. If an employee chooses to maintain benefits, payment for such benefits will be due upon receipt of the related invoice. If payment of such invoice is not received by the Employer within thirty (30) days then benefits will be terminated.

Benefits During Leaves of Absence. X. Xxxx leave accruals shall continue for the entire duration of all FMLA/CFRA leave, military leave and/or as otherwise required by law.
Benefits During Leaves of Absence. ‌ The health and welfare benefits, including health care spending accounts for employees on an approved leaf of absence, may be continued or revoked at their request. Cancellation of benefits must occur within thirty-one (31) days of the onset of the leave. Employees may continue health benefits for the duration of an approved leave of absence; however, the employee is responsible for their share of the insurance premiums. While on leave, the employee’s share of the insurance premiums must be paid by the method normally used during any paid leave. If the employee does not make the premium payments, the Company will have no alternative other than to discontinue your coverage. Employees shall not accrue PTO during the period of leave and are not paid holiday pay while on leave. If benefits are discontinued, you and/or your qualified dependents will be offered continuation of benefits as provided for in the Internal Revenue Code Section 162(k), Consolidation Omnibus Budget Reconciliation Act of 1985 (COBRA). Employer matching contributions to the company 401(k) Plans will not be made during any non-FMLA Leave of Absence. Employees are eligible to resume participation in these plans as provided for in the plan document.

When an employee has qualified for catastrophic leave/continuation of pay status, per Article 18, Leaves, section G.4., and has exhausted all sick leave, extended sick leave, and donated time credits from other employees, but remains unable to work due to that catastrophic event (i.e., the accident or illness that led to the employee being eligible to receive the donated leave) and is placed in unpaid status, the District shall pay the full health and welfare premiums for that employee through the end of the school year in which the leave commenced. In no event shall the District be responsible for more than $5,000, in total, per school year towards premiums.

  • Sick Leave During Leave of Absence (F/T) When an Employee is given leave of absence without pay for any reason, or is laid off on account of lack of work, he/she shall not continue to accumulate sick leave and shall not be entitled to receive pay for sickness for the period of such absence, but shall retain his/her cumulative credit, if any, existing at the time of such leave or lay-off.

  • Unpaid Leaves of Absence A. A department head or designee may grant an unpaid leave of absence for a period not to exceed one (1) year. The employee shall provide substantiation to support the employee's request for an unpaid leave of absence.

  • Other Leaves of Absence In the event that you are on a bona fide leave of absence, not otherwise described in this definition, from which you have not returned to employment with the Company or an Affiliate, your Separation from Service has occurred on the first day on which your leave of absence period exceeds six months or, if earlier, upon your termination of employment (provided that such termination of employment constitutes a Separation from Service in accordance with the last sentence of the first paragraph of this definition).

  • LEAVES OF ABSENCE Article 11 applies for both paid and unpaid leaves. For the purposes of an unpaid 11.25 hour shift, the deduction from pay shall equate to 14.05 hours. For the purposes of an unpaid 7.5 hour shift, the deduction from pay shall equate to 9.375 hours.

  • Union Leaves of Absence In support of the Partnership relationship, upon request, the Employer will grant time off to employees for official union business as long as the number of employees absent for union business does not impose an unreasonable burden on the Employer and the Employer receives reasonable notice. Union leaves will be defined according to the following: Short-Term Leaves are defined as leaves up to 30 days. Employees will continue to accrue seniority, service credit and benefits during the time of the absence, at the expense of the Employer. The impact of multiple short-term leaves on the operations must be considered. Long-Term Leaves are defined as leaves of absence for more than 30 days and up to a maximum of one year. Such leaves will be granted by the Employer in increments of three months and shall be jointly reviewed, on a periodic basis, at the regional level. Seniority, service credit, credited service and health, dental and life insurance benefits will continue during the leave as long as the union reimburses Xxxxxx Permanente for the associated costs.

  • Leaves of Absences 14.01 Written requests for personal leaves of absence without pay for educational or personal reasons will be considered on an individual basis by the nurse’s manager or her designate. Such requests are to be submitted as far in advance as possible with a minimum notice of four (4) weeks except in cases of emergency or if the Employer determines that service needs and staffing requirements can accommodate a leave on shorter notice. A written reply will be given as soon as possible. Such leave shall not be unreasonably withheld. Except in unusual circumstances, a leave of absence other than for educational reasons will not be granted until one year’s continuous service has been completed.

  • Approved Leave of Absence During Vacation Where it can be established by the employee through a doctor's certificate that an illness or accident occurred, or where an employee qualifies for bereavement or any other approved leave during his/her period of vacation, there shall be no deduction from vacation credits for such absence. The period of vacation so displaced shall either be added to the vacation or reinstated for use at a later date, at the employee's option, as mutually agreed.

  • EXTENDED LEAVES OF ABSENCE 1. Military leave without pay shall be granted to any employee who is inducted in any branch of the armed forces of the United States for the period of said service.
