Benefit Provisions. The following benefits will be provided to employees based on eligibility requirements in the local collective agreements:
Benefit Provisions. A. We will pay covered veterinary expenses that you incur during the policy term for the diagnosis or treatment of your pet’s condition, up to the limits of this policy. To be eligible for payment, your pet’s condition or procedure to treat this condition must be listed in the Major Medical Plan Benefit Schedule.
Benefit Provisions. A. We list your deductible and coinsurance percentage on your Declarations Page or Renewal Certificate. Your deductible applies once in each policy term.
Benefit Provisions. 16.2.1 If supervisors are given any options regarding health benefit plans (for example, the choice between Xxxxxx Foundation Health Plan, the District Combined Coverage Medical Plan (PPO) or the District Network Only Plan (EPO) the benefits provided under this section shall be in accordance with the plan or plans under which the retired supervisor was receiving benefits at the time of retirement unless changed during a scheduled open enrollment period. This section shall not be construed as prohibiting any change in benefits or benefit plans as specified in Article 15.
Benefit Provisions. A. We will pay covered veterinary services expenses that you incur during the policy term for the diagnosis or treatment of any covered condition. We will pay up to $600 in veterinary services expenses during the policy term for each covered condition. This is the most that we will pay during the policy term for any covered condition, regardless of the number of incidents or treatments during the policy term.
Benefit Provisions. Section A - Insurance Amount Immediately upon submission of proof of the death of any Insured Member, in a form satisfactory to the Company, an amount, determined in accordance with the Policy Schedule, shall be payable to the designated nominee in the manner herein provided.

Nothing in this Agreement other than the Third Party Benefit Provisions shall be deemed to create any right in any Person (other than Covered Persons) not a party hereto, and this Agreement shall not be construed in any respect to be a contract in whole or in part for the benefit of any third Person (except as provided in Section 30 and except for the Third Party Benefit provisions).
Benefit Provisions. Section 1 Health Insurance
Benefit Provisions. All benefits held by a Teacher at the commencement of a leave without pay may be continued during such authorized leave by the Teacher with the Teacher paying all premium costs by pre-authorized debit. Such coverage normally will be extended to a maximum of one year with further extensions arranged through the Board.
Benefit Provisions. The terms of the Plan shall govern the amount, form, and timing of benefit payments under the Plan to which a Plan participant or beneficiary is entitled.
