Benefit Package. A. The following categories of services shall be provided by the contractor for all Medicaid and NJ FamilyCare Plans A, B, and C enrollees, except where indicated. See Section B.4.1 of the Appendices for complete definitions of the covered services.

Benefit Package. Within three years following a change in control of the Company if your employment by the Employer Company shall be terminated (in circumstances other than normal retirement or death) (a) by the Company other than for Cause, or (b) by you for Good Reason, then you shall be entitled to the following payments and benefits (all as provided in Section 13.05):

Benefit Package. The Benefits within this scheme, to be provided on a cashless basis to the Beneficiaries up to the limit of their annual coverage, package charges on specific procedures and subject to other terms and conditions outlined herein, are the following:
Benefit Package. The benefits offered to the Executive by Qualstar, including but not limited to, Medical, Dental, Life Insurance, Short Term or Long Term Disability, Holidays, 401(K), etc., shall be exactly the same as offered by the Company to other employees. No exclusive top-tier benefit plans shall be offered, as part of Executive compensation.
Benefit Package. The Employer agrees to provide the Association's Benefit package for the general and service employees.
Benefit Package. (a) The Employer agrees to provide a Health Care Benefit Package Assumption Life Contract No: 09775 for employees during the life of this Agreement. The parties by mutual agreement may change the plan carrier at any time during the life of this Collective Agreement.
