Appointment Cancellations. Therapy is a commitment. It is a collaborative relationship where each of us has a responsibility to the process, and it’s important for your healing and the therapeutic process that you attend your sessions regularly. I will reserve and hold a one hour weekly time slot for us to meet. This means that we will meet at a set time each week unless you cancel your appointment. I understand that unforeseen events can sometimes prevent you from attending your prearranged appointment time. If you wish to cancel or re- schedule an appointment, two days (specifically 48 hours) notice will need to be given. If you cancel or don’t show up for an appointment with less than 48 hours notice you will be responsible for full payment of the session. If you are running late for your appointment you can call me at (000)000-0000 to let me know of your expected arrival time. I will wait 15 minutes after your scheduled appointment time. If I do not hear from you, your appointment time will be forfeited and you will be charged the full session fee. TELEPHONE CALLS: Telephone messages can be left at (000)000-0000. I will attempt to return your call within a few hours. No charge will be made for short calls; however, you will be charged for lengthy phone sessions. In the unlikely event that you cannot reach me, and your call is a serious medical emergency, please call 911, or visit your local emergency room.

By asking the Team Member to perform these services, the Client agrees that the time spent shopping, researching, transporting Client equipment, or traveling to and from any location (such as a repair or retail shop) will be deducted from the Service Term’s hours, or charged the normal hourly rate. If The Company or any Team Member is purchasing any item for the Client (including computer parts), they will be invoiced for the cost of the item plus a 30% markup. Monitoring Limitations: If the Client purchases Monitoring, the Company will alert the Client regarding any issues deemed critical, based on our professional judgement. Because the monitoring software is not created or maintained by the Company, the Company cannot assume responsibility for any missed malware, viruses, or system failures the Monitoring does not detect. We also are not responsible for alert emails that are misdirected or do not reach the Client. Refunds, Expiration, Holds and Service Term Cancellations: Refunds will not be issued. All services and discounts are determined on the first day of the Service Term (the date on this signed Agreement). If the Client fails to make any and/or all payments on time and in full during their Service Term, the Company will invoice the Client for all one-on-one hours and Monitoring services accrued to date at the full and non-discounted rates, which the Client agrees to pay upon receipt. Service Hours are fully transferrable, however all hours expire at the end of their Service Term. Clients may extend their date on expiring hours by renewing or upgrading the Service Term subscription within Fourteen (14) days of the expiration date. Appointment Cancellations. We understand that sometimes plans change. We ask that you reschedule appointments at least 24 hours in advance so that we may give that time to someone else. Although unexpected events may necessitate missing an appointment, if you miss 2 appointments without following the cancellation protocol then you will be charged $40.00. If you miss 3 appointments without following the cancellation protocol, you may be dismissed from the practice. You will receive a written notification if you miss 2 appointments.
