Application Review Information. 1. EVALUATION CRITERIA The Project Narrative describes what you intend to do with your project and includes the Evaluation Criteria in Sections A-D below. Your application will be reviewed and scored according to the quality of your response to the requirements in Sections A-D. • In developing the Project Narrative section of your application, use these instructions, which have been tailored to this program. • The Project Narrative (Sections A-D) together may be no longer than 25 pages. • You must use the four sections/headings listed below in developing your Project Narrative. You must indicate the Section letter and number in your response or it will not be considered, i.e., type “A-1”, “A-2”, etc., before your response to each question. Your application will be scored according to how well you address the requirements for each section of the Project Narrative. • Although the budget and supporting documentation for the proposed project are not scored review criteria, the Review Group will consider their appropriateness after the merits of the application have been considered. (See PART II: Section V and Appendix F). • The number of points after each heading is the maximum number of points a review committee may assign to that section of your Project Narrative. Although scoring weights are not assigned to individual bullets, each bullet is assessed in deriving the overall Section score.

Application Review Information. In order to receive a cooperative agreement under this FOA, applicants must submit an application, in the required format, no later than the deadline date. If an applicant does not submit all of the required documents and does not address each of the topics described below, the application will be disqualified. As indicated in Section IV. 2, Content and Form of Application Submission, all applicants must submit the following:

Criteria Eligible recipients will be evaluated against the following criteria: ASPR and CDC will evaluate the extent to which the applicant: • Presents outcomes that are consistent with the project period outcomes described in Maximum Points: 35 Approach the ASPR-CDC Project Description and logic model. • Describes an overall strategy and activities consistent with the ASPR-CDC Project Description and logic model. • Describes strategies and activities that are achievable, appropriate to achieve the outcomes of the project, and evidence-based (to the degree practicable). • Shows that the proposed use of funds is an efficient and effective way to implement the strategies and activities and attain the project period outcomes. • Presents a work plan that is aligned with the strategies/activities, outcomes, and performance measures in the approach and is consistent with the content and format proposed by ASPR and CDC. ASPR and CDC will evaluate the extent to which the applicant: • Shows/affirms the ability to collect data on the process and outcome performance measures specified by ASPR and CDC in the project description and presented by the applicant in their approach. • Describes clear monitoring and evaluation procedures and how evaluation and performance measurement will be incorporated into planning, implementation, and reporting of project activities. • Describes how performance measurement and evaluation findings will be reported, and used to demonstrate the outcomes of the FOA and for continuous program quality improvement. • Describes how evaluation and performance measurement will contribute to developing an evidence base for programs that lack a strong effectiveness evidence base. • Includes a preliminary Data Management Plan (DMP), if applicable.
