Annual Activity Report. All Members holding probationary, tenured, continuing or term appointments shall submit an annual activity report on the appropriate form set out in the Appendices to the Agreement to their Chair/Director or Xxxx of the Library/Administrator by October 1 of each year. The activity report shall include the activities during the academic year ending August 31 in relation to the evaluation criteria agreed upon accordance with Clause 14.19(2), and an indication of goals and expected outcomes for the subsequent evaluation period. Notwithstanding Clause 23.22(9), new appointees with fewer than six (6) months of service in the academic year need not submit an activity report. A copy of this report shall be filed in the office of the Chair/Director or Xxxx of the Library/Administrator and the original shall be placed in the Member's personnel file.

Annual Activity Report. A cumulative Plan year statistical report delivered during the Contract Term to parties identified by the County within thirty (30) days after the end of each Plan year. If the Contractor fails to deliver the Annual Activity Report within the specified time period, the Contractor will pay the County $500 per day.

Notwithstanding Clause 25.17(8), new appointees with fewer than six (6) months of service in the academic year need not submit an activity report. A copy of this report shall be filed in applicable Academic Administrator’s office and the original shall be placed in the Member's personnel file.
Annual Activity Report. By October 1 of each year, Members shall submit their AAR to the Clinical Coordinator and the Executive Director. The Clinical Coordinator shall submit their AAR to the Executive Director.

Each Member shall submit to his/her Chair/Library Department Head/Coordinator or Associate Vice-President (Student Services) an activity report by November 15th of the year of the performance evaluation. The activity report shall include the activities during the calendar year(s) being evaluated. A copy of this report shall be filed in the office of the Chair/Library Department Head/Coordinator or Associate Vice-President (Student Services) and the original shall be placed in the Member's personnel file. The Departmental Personnel Committee shall determine a method of distributing the reporting time periods for new Members with tenure or continuing appointments. Members with tenured or continuing appointments who are subject to a bi-annual activity report and evaluation may file an activity report covering the first year of the two (2) year period. This activity report shall be forwarded to the Member for inclusion in the personnel file and will not be evaluated by the DPC until the end of the bi-annual evaluation period. The early filing of a one (1) year activity report is for the convenience of the Member and does not constitute a request for an annual activity report and evaluation.
