Amendments to Protected Health Information. The Agency agrees to make any amendments to Protected Health Information that the County directs or agrees to pursuant to 45 C.F.R. § 164.526 within ten (10) business days of the County's request.

Business Associate agrees to make any amendment(s) to Protected Health Information in a designated record set, if applicable, that Covered Entity directs or agrees to pursuant to 45 C.F.R. 164.526, at the request of Covered Entity or an Individual, and in a reasonable time and manner.

Within ten (10) days of a request by Covered Entity, Business Associate shall provide Protected Health Information in its possession or in the possession of a Subsequent Business Associate to Covered Entity in order for Covered Entity to comply with its obligations under 45 C.F.R. 164.526 to provide Individuals the right to amend their Protected Health Information.

Employee must act on an individualís request for an amendment in a manner and within the time period set forth in 45 C.F.R. ß 164.526(b)(2).
Amendments to Protected Health Information. If Business Associate maintains PHI for Covered Entity in a designated record set, Business Associate will make such PHI available to Covered Entity within 14 days of request from Covered Entity to permit Covered Entity to meet its obligations under 45 C.F.R. § 164.526, and Business Associate will incorporate any amendments made by Covered Entity to such PHI.

Contractor must act on an individual’s request for an amendment in a manner and within the time period set forth in 45 C.F.R. § 164.526(b)(2).
Amendments to Protected Health Information. 2.6.1 If Provider requests that Marquette make any amendment(s) to Protected Health Information in a Designated Record Set, then Marquette will forward the relevant records to Provider for amendment and accept and incorporate such amendment(s) in the Protected Health Information as Provider directs or agrees to make pursuant to 45 CFR § 164.526.
Amendments to Protected Health Information. Neoforma agrees to make any amendment to an individual’s Protected Health Information upon the reasonable request of the Covered Entity so that the Covered Entity can meet its obligations under 45 CFR § 164.526.
