AIMS AND OUTCOMES. The main purpose of integration is to improve the wellbeing of people who use health and social care services, particularly those whose needs are complex and involve support from health and social care at the same time. The Integration Scheme is intended to achieve the National Health and Wellbeing Outcomes. Argyll and Bute Integration Joint Board (IJB) will plan for and deliver high quality health and social care services to, and in partnership with, the communities of Argyll and Bute. The IJB will set out within its Strategic Plan how it will effectively use allocated resources to deliver the National Health and Wellbeing Outcomes prescribed by the Scottish Ministers in regulations under section 5(1) of the Public Bodies (Joint Working) (Scotland) Act 2014, namely that: • People are able to look after, and improve, their own health and wellbeing and live in good health for longer. • People, including those with disabilities or long term conditions or who are frail are able to live, as far as reasonably practicable, independently and at home or in a homely setting in their community. • People who use health and social care services have positive experiences of those services, and have their dignity respected. • Health and social care services are centered on helping to maintain or improve the quality of life of people who use those services. • Health and social care services contribute to reducing health inequalities. • People who provide unpaid care are supported to look after their own health and wellbeing, including reducing any negative impact of their caring role on their own health and wellbeing. • People using health and social care services are safe from harm. • People who work in health and social care services feel engaged with the work they do and are supported to continuously improve the information, support, care and treatment they provide. • Resources are used effectively and efficiently in the provision of health and social care services. • Any other National Health and Well Being outcome prescribed in the future will also be adopted. This agreement aims to provide the fitting and removal element of LNG-IUS for women requiring Levonorgestrel intrauterine system LNG-IUS fitting as management of menorrhagia or other gynecological purpose such as HRT, endometriosis etc. where clinically relevant, thus reducing the requirement for hysterectomy.

AIMS AND OUTCOMES. The purpose of Transformation Fund is to provide incremental non-recurrent resources to enable systems to transform the care provided to the residents and patients of Oldham. The Partners’ aim is that people in Oldham will be independent, resilient and self-caring so fewer people reach crisis point. For those that need it, the Partners will commission or design integrated health and care services that enable people to proactively manage their own care as far as they are able, with the support of their family, community and the right professionals at the right time in a properly joined up system. All commissioning activity will reflect the high-level outcomes of the Integrated Care Organisation (ICO) and translate these into meaningful, relevant and measurable personal or population level outcomes.

AIMS AND OUTCOMES. 1. The Parties wish to use this Agreement to enable the Lead Commissioning arrangements for the Services, which have been categorised into 6 designated service areas, as listed in Clause 5 (Services) of the Agreement and more specifically detailed in Schedule 5 (The Services).

The intention is to jointly oversee strategic commissioning, through a population-based approach, focused on health and social care outcomes for all adults with a learning disability. The commissioning of services for people with a learning disability will be transformed through joint commissioning budgets and contracting arrangements, which incentivise system-wide improvement, placing ever-greater emphasis on the outcomes achieved in addition to the quantity of activity delivered.
