AGREEMENT IN MULTIPLE COUNTERPARTS. The parties hereto agree that this Agreement may be executed in one or more counterparts, each of which shall, for all purposes, be deemed an original and all of such counterparts, taken together, shall constitute one and the same Agreement. XXXX COUNTY COMMUNITY DEVELOPMENT PROGRAM Exhibit A: Project Description, Tasks, Schedule, and Budget Project Description and Tasks The Subrecipient will operate a year-round emergency shelter that serves homeless persons from the County’s Community Development Program Area, which includes all of Xxxx County except the municipalities of Algonquin, Aurora, Elgin, Hampshire, Xxxxxxx Estates, Huntley, and Xxxxxxxxxx. The anticipated number of persons from the County’s Community Development Program Area to be served during the term of this Agreement is 165. The Subrecipient shall be responsible for all tasks required to complete the project described herein. Additional details concerning the project are described in the Subrecipient’s application, which is on file with the County and incorporated into this Agreement by reference.

KANE COUNTY COMMUNITY DEVELOPMENT PROGRAM Exhibit A: Project Description, Tasks, Schedule, and Budget Project Description The Subrecipient will complete street resurfacing and sidewalk improvements along Tee Lane and Tee Road in the Village of Carpentersville. Specifically, the project will involve the select removal and replacement of existing concrete carriage walks with concrete curbs/gutters and concrete sidewalks behind and adjacent to the new curbs, and the removal/replacement of 2” of existing asphalt street pavement. The project will also include all necessary base repairs, new pavement markings, and ADA upgrades at intersections. Said work will be financed in part with CDBG funds provided herein. Additional details concerning the project are described in the Subrecipient’s application, which is on file with the County and incorporated into this Agreement by reference.

KANE COUNTY/CITY OF ELGIN CONSORTIUM HOME INVESTMENT PARTNERSHIPS PROGRAM Exhibit A: Project Description, Tasks, Schedule, and Budget Project Description The Subrecipient will provide housing rehabilitation services to approximately 6 low-income, owner-occupied households in the Consortium Area, which includes all of Kane County (except the municipalities of Algonquin, Aurora, Hampshire, Xxxxxxx Estates, Huntley, and Xxxxxxxxxx) and all of the City of Elgin, including that portion located in Cook County. All households assisted shall be at or below 80% of the Area Median Income for the Chicago PMSA (Primary Metropolitan Statistical Area), as established and adjusted annually by HUD. HOME funds will be used by the Subrecipient to rectify code violations and address substandard living conditions, and to pay for eligible soft costs and program-delivery expenses. Up to $20,000 in assistance will be provided to each household to cover rehabilitation hard costs. Such assistance will be provided as an interest-free, deferred-payment loan, which shall be secured by a mortgage payable to Kane County. The subordination of loans provided herein shall comply with the guidelines contained herein (see Exhibit C). In housing units with lead-based paint hazards, an additional $10,000 will be made available as a grant to pay for lead-hazard control work. In households where additional funds are required to complete necessary lead-hazard control work, the amount of assistance provided as a loan for rehabilitation hard costs may be reduced so that additional assistance may be provided as a grant for lead-hazard control costs. In no event, however, shall the total amount of assistance provided to a household (loan amount plus grant amount) exceed $30,000. Eligible soft costs include title searches and lab costs. Additionally, the Subrecipient shall be entitled to a program delivery fee of twenty percent (20%) of eligible hard rehabilitation costs and lead- hazard control costs.

Said work will be partially financed with CDBG funds provided herein which will be secured by a recorded twenty-year forgivable mortgage and promissory note. Additional details concerning the project are described in the Subrecipient’s application, which is on file with the County and incorporated into this Agreement by reference.

Said project will be financed in part with CDBG funds provided herein. Additional details concerning the project are described in the Subrecipient’s application, which is on file with the County and incorporated into this Agreement by reference.

The anticipated number of persons from the County’s Community Development Program Area to be served during the term of this Agreement is 200. The Subrecipient shall be responsible for all tasks required to complete the project described herein. Additional details concerning the project are described in the Subrecipient’s application, which is on file with the County and incorporated into this Agreement by reference.

Specifically, the project will involve the abandonment of existing watermains and service lines located in back yards, the construction of new 8-inch watermains within the rights-of-way of each street, and the installation of new fire hydrants. Said work will be financed in part with CDBG funds provided herein. Additional details concerning the project are described in the Subrecipient’s application and subsequent correspondence and submittals to the County, which are on file with the County and incorporated into this Agreement by reference.
