BUSINESS ASSOCIATE AGREEMENT This Business Associate Agreement ("BAA") is entered into by and between Chino Valley Independent Fire Agency ("Business Associate"), a fire protection district organized pursuant to the Fire Protection District Law (Health & Safety Code §13800, et seq.) and the Consolidated Fire Agencies ("Covered Entity"), a California joint powers authority existing pursuant to Gov. The Company agrees to pay the Employee an Annual Consulting Fee for services provided up to the Maximum Hour Allotment for each Consulting Year as set forth below. The Annual Consulting Fee shall be payable in twelve (12) equal monthly installments, in arrears, whether or not the Company requires the Employee to provide services up to the Maximum Hour Allotment for the Consulting Year. If the Company requests the Employee to provide services that exceed the Maximum Hour Allotment for any Consulting Year, and the Employee agrees to do so, upon submitting reasonable documentation of the same, the Company agrees to pay the Employee $275 for each hour of service provided in excess of the Maximum Hour Allotment for the Consulting Year.
