Central Region Mr. Xxxxxxx Xxxxx, Regional Maintenance Engineer 000 Xxxxxxx Xxx Freehold, NJ 07728 Telephone: 000-000-0000 SECTION 104 – SCOPE OF WORK 104.01 INTENT THE FOLLOWING IS ADDED: The work to be performed under this contract shall consist of the repair of the roadway surface to restore it to a level which will improve rideability, and the removal of any unsound or locally deteriorated pavement and placement of approved hot mix asphalt material on the areas designated by the RE. The proposed treatment shall consist of milling the roadway surface and resurface with Hot Mix Asphalt 12.5M76 Surface Course or Hot Mix Asphalt 9.5H64 Surface Course as designated by the RE, unless otherwise indicated. Quantities and/or locations may be adjusted by the RE and required completion time will be adjusted accordingly. The Department reserves the right to perform any emergency roadway repair operation within the limits of this contract whenever the department determines that such work is in the public’s interest and safety. The minimum request for “if and where directed” locations will be for the amount in which the aggregate total is $10,000.00. This shall not be construed to imply that each site will be for the amount of $10,000.00. Multiple sites may be submitted in a single request, having an aggregate total of at least $10,000.00. Multiple sites within a single request must be within a 5 mile radius. Additional sites within the request may be identified during the 10 day time period described in Subsection 105.08 Cooperation by Contractor or within the time frame of the construction activities for the request. In no case shall a site be added to an existing request if the site had been identified subsequent to the Contractor removing construction equipment from the 5 mile radius. The radius will be determined by measuring from the center of the initial site on the request. Only those locations on the same highway, or intersecting highways by means of interchanges or at-grade intersections within the 5 mile radius will be constructed as being a single request. The item HMA Patch shall only be used in those areas that were previously milled as a part of this contract and found to be unstable, or, in those areas that were not previously milled as a part of this contract, but are adjacent to and within a distance of 55 yards of the limits of milling.
