The purpose of the Transition Care Conference is to ensure timely and sound transition planning. Transition Care Conference participants will:
(1) Identify Department or AAA and Contractor transition planning roles and responsibilities;
(2) Identify guardian, representative xxxxx, and designated representative assignments;
(3) Identify primary care physician and other health care provider(s);
(4) Identify Individual’s transition needs to include but not limited to: DME, medications, transportation, supplies, ancillary services, etc.;
(5) Review medical needs with a plan to ensure coordination of medical benefits and services; and
(6) Review existing service or plans and identification of staffing needs.

d) The UTSA Admissions office, based on anticipated yield and the holistic review of applications and all relevant application documentation, will invite a percentage of these talented students to participate in the XXXX.
e) Students will be encouraged to complete their associate degree from Xxxxx College. However, an option to apply for early entrance into a XXX degree granting major is available for TATE students who have completed a minimum of two math, two science, and two engineering courses approved by the XXX, and have earned a minimum 2.5 Cumulative Grade Point Average (COPA) at both institutions as calculated by UTSA by the end of the term in which they submit an application. The option to apply for early entrance into a XXX degree granting major is available each fall and spring term beginning with the students second semester.
x. XXXX students will complete the entire calculus sequence required for the admitted major prior to the semester of full enrollment.
x. XXXX students who receive admission into a XXX degree granting major prior to completion of the two-year program may retain their offer of admission while continuing/completing the two-year co-enrollment program.
