Admission Process. ESHC agrees that it and its physicians and/or medical staff will abide by the following notification procedures when patients are transported to Shore Health: the sending physician at ESHC shall contact the appropriate Emergency Department attending physician at Shore Health who will evaluate the patient and determine appropriate disposition. In the event there is one (1) ESHC physician treating two injured patients that require emergency care and no additional physician coverage at ESHC is available to such physician, the patient with the most serious injuries will be sent to the Emergency Department, and the patient with less serious injuries will be managed at ESHC; provided that ▇▇▇▇ shall utilize best efforts to notify Shore Health of such transfer in advance via telephone.

Admission criteria will be determined by referral source, Contractor, and/or funding stream. C. Admission Process. (a) Additional Members may join the Company upon the vote of the Board, subject to Section 9.5(b)(ii). (b) Any Person who shall acquire Membership Interests pursuant to this Section 15.1 as an Additional Member shall as a condition to acquiring such Membership Interests be required to execute a counterpart of, or an agreement adopting, this Agreement. Upon execution of such counterpart or agreement, the acquiring Person shall have, to the extent of the Membership Interest acquired, the rights and powers and shall be subject to the restrictions and liabilities of a Member under this Agreement, the Certificate of Formation and the Act. Notwithstanding anything in this Agreement to the contrary, an Additional Member (including any Permitted Transferee or other Transferee of a Founding Member) other than a wholly-owned subsidiary of a Founding Member, shall not be entitled to any of the rights and powers reserved expressly for a Founding Member under any circumstances (excluding, for the avoidance of doubt, any Transfer solely in connection with a Change of Control of the Company otherwise approved in accordance with Section 9.5(b)(v) and the other terms of this Agreement).

Contractor shall admit only those clients referred by sources described in Section 8 (Referrals) and whose substance use issues and treatment needs are within the scope of the Contractor's practice, as follows: . i. Determination of client's eligibility for enrollment in the Program shall be made by the VETS Court Team based on admission criteria established by the VETS Guidelines. ii. Contractor shall interview and screen client to confirm client's appropriateness for the Program.

All Medicaid admissions under this contract must be approved by APD Central Office prior to admission. c. The Contractor will screen all Individuals being considered for placement under this Contract and review screening results and all related service planning information with relevant Service Planning Team members including the DHS Designee prior to establishing a targeted admission date. d. The Contractor and the DHS Designee will mutually determine the targeted admission date and mutually confirm the actual admission date after receiving confirmation of the Department’s final approval. e. The Contractor shall coordinate and participate in a minimum of one Transition Care Conference prior to the targeted admission date with the DHS Designee of both the referring and receiving Department or AAA branch and a representative of the provider(s) currently providing services to the Individual (as applicable). The purpose of the Transition Care Conference is to ensure timely and sound transition planning. Transition Care Conference participants will: (1) Identify Department or AAA and Contractor transition planning roles and responsibilities; (2) Identify guardian, representative ▇▇▇▇▇, and designated representative assignments; (3) Identify primary care physician and other health care provider(s); (4) Identify Individual’s transition needs to include but not limited to: DME, medications, transportation, supplies, ancillary services, etc.; (5) Review medical needs with a plan to ensure coordination of medical benefits and services; and (6) Review existing service or plans and identification of staffing needs.
