Admission Agreement. D13.1 You must ensure that each Resident, or their nominated representative, signs an Admission Agreement, either on the day that the Resident commences receiving Services at your Facility, or, in the case of an Emergency Admission, as soon as is reasonably practicable but no later than 10 Working Days after the Resident is admitted. You must not charge the Resident or any other Person for preparing or providing an Admission Agreement.
Admission Agreement. The hospital admission agreement signed by the patient or the patient’s legal representative shall contain the following statement: Pathology and radiology services are medical services performed or supervised by doctors, and the personnel and facilities are or may be furnished by the hospital for said services. Charges for such services are or may be collected, however, by the hospital on behalf of said doctors pursuant to an agreement between said doctors and the hospital, and from said charges I consent that an agreed sum will be retained by the hospital in accordance with an existing agreement between the doctor and the hospital. Admission Agreement. Should the resident be required to temporarily leave the facility for a therapeutic leave or hospitalization, the terms and conditions of the resident’s original admission agreement shall remain in effect during the course of any bed hold period (or lack thereof) and remain in effect until the resident’s readmission to the facility. Admission agreements do not remain in effect once a resident is officially discharged from the facility. Hospital or therapeutic leaves that result in official discharge may require the execution of a new admission agreement.
Admission Agreement. The written individual admission agreement 7 between the CONTRACTOR, the Xxxxxx Youth/Non Minor Dependent (NMD) and Xxxxxx 8 Youth’s/NMD’s authorized representative as required by Title 22, California 9 Code of Regulations Section 80068.

Admission Agreement. At the time of or prior to your admission you will be asked to sign an admission agreement. The Admission Agreement is a legally binding contract that defines the rights and obligations of each person (or party) signing the contract. The admission services manager or other facility staff member will take time to review the Admission Agreement with you. Please feel free to ask questions until you are satisfied that all of your concerns have been adequately addressed. At the time of your admission please bring the following items: □ Social Security Card □ All Insurance Cards □ Advanced Directive, including Health Care Power of Attorney (if you have appointed someone for health care) and Living Xxxxx, and/or POLST (if you have prepared one) □ Power of Attorney to handle all your medical or financial needs □ Correct name, address, and phone number of those individuals you would like listed as contacts This information helps us determine the correct payment source and ensures we have accurate contact information in the resident file. Clothing and Personal Items You or your family are responsible for providing clothing. We encourage residents to dress in regular clothing if possible, and we recommend that you bring several changes of clothing. Be sure to include active wear such as sweats and tennis shoes that may be worn during therapy. We strive to protect and preserve the personal property of each of our residents and recommend items are marked with your name for easy identification. Clothing items requiring dry cleaning are the responsibility of the resident. At the time of your admission we will give you the option to have laundry cleaned by your family, the facility, or an outside laundry service. Please keep in mind the facility uses industrial detergents in commercial machines on very high heat. We recommend not bringing clothing items that are delicate or cannot tolerate high heat. All clothing and other personal items should be listed on the inventory check list when you are admitted. Please notify the charge nurse of any additional items brought into the facility and any items taken out of the facility.
Admission Agreement. The Authority shall before the Relevant Service Transfer Date execute each of the Admission Agreements referred to in Clause 26.1 (DBFM Co to become an Admission Body) [and shall use reasonable endeavours to ensure that the Administering Authority executes each such Admission Agreement before the Relevant Service Transfer Date10]. Indemnity for a breach of the Admission Agreement DBFM Co hereby indemnifies the Authority and/or any future Service Provider and, in each case, their sub-contractors on demand from and against all Direct Losses suffered or incurred by it or them which arise from any breach by DBFM Co (or any relevant Service Provider) of the terms of the Admission Agreement to the extent that such liability arises before or as a result of the termination (howsoever caused), suspension or expiry of this Agreement. Indemnity or bond DBFM Co shall procure that it (and any relevant Service Provider) shall as soon as reasonably practicable obtain the indemnity or bond required in accordance with the Admission Agreement.11
