Common use of Address Changes Clause in Contracts

Address Changes. An employee shall notify the Employer in writing of any change in name, telephone number, or address promptly and, in any event, within five (5) days after such change has been made. The Employer shall be entitled to rely upon an employee's last name, telephone number, and address shown on his/her record for all purposes involving his/her employment.
