High Deductible Health Plan (HDHP) • Participation is mandatory for new hires, and teachers participating in the HDHP must remain in the HDHP. • Prescription co-pays ($5/$30/$45) apply after the deductible is satisfied. Preferred Provider Organization (PPO-In Network) There is no annual deductible cost to the individual, no family deductible, and no co-insurance costs for those expenses incurred within the Network of doctors and hospitals. This Plan has no lifetime maximum if services are provided in Network. Out of the HMO Network the individual has total responsibility for medical expenses, except in an emergency.
