Acknowledgment of Risk. I recognize and appreciate the dangers, hazards, and risks associated with participation in the camp activities. I understand that the dangers, hazards, and risks of the camp activities could include serious or even fatal injuries and property damage. I acknowledge that I have fully considered the dangers, hazards, and risks associated with my/Camper’s participation in the camp activities, and voluntarily assume those dangers, hazards, and risks. I give my consent and approval for my/Camper’s participation in the camp activities.
Acknowledgment of Risk. The Client expressly acknowledges and agrees that the use of electronic communication systems to convey Instructions does not eliminate the risk of error and fraudulent activities or security and privacy issues.
Acknowledgment of Risk. Participant acknowledges and agrees that FMU has made no warranty, express or implied, concerning the safety of Participant during the Mission Trip and further acknowledges and agrees that he/she is aware that Mission Trips of this nature pose risks including, but not limited to, accident, injury, disease, crime, political instability, religious intolerance and hostility, and government opposition to project activities. Participant acknowledges and agrees that when working, visiting or undertaking activities in underdeveloped economies and/or countries, there is a potential for problems with lack of sanitation, law enforcement protection, and health standards for local and foreign individuals, and that overt and/or covert discrimination against U.S. citizens, Christians, or persons of a particular ethnic background may be encountered, and that it cannot be assumed that danger or harm will not occur.
Acknowledgment of Risk. I understand and acknowledge that I voluntarily engage in the Course as a participant and, therewith, accept all risks, known and unanticipated, which could result in my injury, death, illness, disease, physical or mental, or damage to myself, to my personal property or to third parties. I understand and acknowledge that those risks may result in personal claims against Ah Haa or claims against me by third parties. Among these risks are the following:

I understand that such risks cannot be eliminated without jeopardizing the essential qualities of these activities. I also understand that I can cease exposure to such risks by choosing not to partake in the activities or by otherwise withdrawing from such activities at any time. I acknowledge that the guides, instructors and facilitators have difficult jobs to perform and that although they seek safety, they are not infallible. I am aware that they may give inadequate warning or instruction and that the inherent and other risks of the activities that I am about to perform may be increased by my own fitness or ability levels, by weather or other environmental conditions, by equipment malfunction, structural failure and other unforeseen factors that may affect my overall safety. I acknowledge that the property on which these activities take place include their own inherent and other risks including, but not limited to, rocky and wooded terrain with potentially harmful plants and animals, including snakes and other creatures which may bite or sting. Acknowledgment of Risk. (i) The Purchaser acknowledges and understands that its investment in the Note and Conversion Shares involves a significant degree of risk, including, without limitation, (A) an investment in the Debtor is speculative, and only Purchasers who can afford the loss of their entire investment should consider investing in the Debtor and the Note and Conversion Shares; (B) the Purchaser may not be able to liquidate its investment; (C) transferability of the Note and Conversion Shares may be limited; (v) in the event of a disposition of this Note or the Conversion Shares, the Purchaser could sustain the loss of its entire investment; and (D) the Debtor has not paid any dividends on its Common Stock since inception and does not anticipate the payment of dividends in the foreseeable future; and
Acknowledgment of Risk. You understand and acknowledge that your travel in connection with and participation in the tour arranged at your request by QEI may involve risk and potential exposure to injury, including emotional injury, injury to person or property and death. You also realize and acknowledge that risk and dangers may be caused by the negligence, fault or wrongdoing of the owners, directors, employees, contractors, subcontractors, officers or agents of QEI or of other participants, contractors and/or subcontractors to QEI. You also recognize and acknowledge that risk and dangers may arise from foreseeable and unforeseeable causes. You fully understand and acknowledge that the aforementioned risks, dangers and hazards are a potential in connection with your travel and recreational activities which may take place during your journey.
Acknowledgment of Risk. I understand that participation in C.R.E.W. programs will include participation in a variety of group activities as well as possible travel. Activities will take place in a public setting, in which those coordinating the activity will, in many cases, not have control over the environment or surroundings. The risk of injury from the activities involved in these sport events is significant, including the potential for permanent paralysis and death, concussion, possible broken bones along with minor scrapes and bruises, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my participation; and I willingly agree to comply with the stated instructions and policies and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official/coach/instructor immediately; and I understand, agree and acknowledge that some activities may be of a hazardous nature and/or include physical and/or strenuous activity. Understanding this, I state that I have no medical condition or impairment that might inhibit my safe and active participation in the above listed activity. It is my understanding that the risks associated with the activity include all of the risks associated with the activities described above as well as other risks that may arise during my participation in the activity. Accordingly, if I believe I need more information regarding the participation in said activity before I waive my rights and agree to indemnify others as described below, I acknowledge that I may seek additional information or I may refuse to participate in the activity. I further acknowledge that the risks listed above, along with other risks inherent to participation in the activity listed above, may result in severe bodily and/or emotional injury, including permanent disability or death. I recognize that participation in the above listed activity involves other inherent risks, and that all such risks cannot be described as a part of this document.
Acknowledgment of Risk. Guardian acknowledges that the nature of the Activity may expose Minor to hazards or risks that may result in personal injury, illness, or death, caused by: (a) engaging in the Activity or events associated with Activity; (b) traveling via any means of transportation to or from the Activity or during the duration of the Activity; (c) exposure to or use of materials, tools, supplies, equipment, machinery, or other items that are associated with or utilized during the Activity or related activities; or (d) exposure to other dangerous conditions associated with the Activity.
