Accounting Log. Maintain an ongoing log of the details relating to any disclosures of PHI it makes (including, but not limited to, the date made, the name of the person or organization receiving the PHI, the recipient’s address, if known, a description of the PHI disclosed, and the reason for the disclosure). Upon written request make available the information required to provide an accounting of disclosures to either the CSB or Individual as necessary to satisfy the CSB’s obligations under 45 CFR 164.528; If an Individual makes a written request for an accounting of disclosures directly to the Business Associate as provided in 42 U.S.C. § 17935(c)(3), the Business Associate shall provide such accounting to the Individual pursuant to 42 U.S.C. § 17935(c)(3). The Business Associate shall, within thirty (30) days of the CSB’s request, make such log available to the CSB, as needed for the CSB to provide a proper accounting of disclosures to its patients.

  • Accounting Format Applications for Payment shall be broken down by CSI Category and, in certain situations, by CSI Description and capital asset category, as set forth in the form for Application for Payment.
