Accounting for Leave. An employee who is absent or who takes, or proposes to take, any leave as provided for in this article must sign and forward the approved leave form provided by the Board to the Superintendent or his/her designee. The employer shall provide to each employee, no later than the day of the second pay of each month, a statement of the employee's accumulation of sick leave, personal leave and vacation leave, which shall be accurate through the last day of the preceding month.

Accounting for Leave. 1. Employees are responsible to submit leave requests to cover FMLA absences and indicate that the leave is being taken under FMLA on the leave request form. The FMLA leave codes for sick, vacation, personal holiday and leave without pay are listed on the leave request form.

  • Compensation for Holidays Worked a. With the exception of the provisions in Section C.2.b., below, an employee required to work on a holiday listed above shall be paid at the employee's regular straight-time rate of pay for the hours actually worked. In addition, an eligible employee shall receive either compensatory time off or holiday pay at the option of the University at the regular straight- time rate, including any shift differential.

  • Basis for Layoff A. The reasons for layoffs include, but are not limited to, the following:

  • Application for Leave of Absence 25.01 All leaves of absence without pay and any extension thereof must be applied for in writing to the Mayor or his designee, on forms supplied by the Employer, at least ten (10) working days prior to the proposed commencement of the leave except in serious or unusual circumstances, as determined by the Employer. Notification of the approval or denial of their requested leave shall be given to the employee in writing within five (5) working days after the submission of the request. Any denial of a requested leave shall include the reason for the denial.

  • Annual Leave Exclusive of Public Holidays The period of annual leave prescribed by this clause is exclusive of any public holidays, and if any such holiday falls within an employee’s period of annual leave and is observed on a day which in the case of that employee would have been an ordinary working day, there will be added to the period of annual leave time equivalent to the ordinary time which the employee would have worked if such day had not been a holiday.
