Absence Reports. Workers must complete an absence report within five (5) working days upon return to duty. In the case of extended illness, the supervisor will complete the absence reports and send the original to Human Resources and a copy to the worker.

Absence Reports. Reports of personal illness resulting in the use of sick leave shall be directed to the school. If an absence that may continue is anticipated, the teacher shall attempt to notify the school secretary before 2:30 p.m. of the need to continue on sick leave. Verification of illness may be required by the District pursuant to Board Policy/Administrative Regulation 4161.1.
Absence Reports. An employee, part or full-time, is not expected to be absent from work for any reason other than personal/family illness or emergency situations without making prior arrangements before the start of his/her shift with the supervisor. Employees finding it necessary to be absent from work are to notify their immediate supervisor, or a designee, immediately. If the absence continues beyond the first day the employee must notify the supervisor on a daily basis unless under long-term physician's care in which case a notice from the Physician/Physician's Assistant will be required.
Absence Reports. 18.4.1 The Contractor shall orally report the absence of any Security Officer from an assigned post or position to the Director. This report shall not be made later than five minutes after the start of the reporting time for the post or position. The report shall include the name of the absent Security Officer, the post or position affected and Contractor's efforts to meet the security needs of the City. A relief officer or supervisor shall immediately fill the vacancy until other arrangements are made.

  • Usage Reports The Contractor shall submit usage reports on an annual basis to the assigned Deputy Purchasing Agent of the County of Orange user agency/department. The usage report shall be in a format specified by the user agency/department and shall be submitted 90 days prior to the expiration date of the contract term, or any subsequent renewal term, if applicable.

  • Adverse Reports The College shall notify an employee in writing of any expression of dissatisfaction concerning his/her work within ten (10) working days of the event of the complaint, with a copy to the Chair of the Union Standing Committee. This notice shall include particulars of the work performance which led to such dissatisfaction. If this procedure is not followed, such expression of dissatisfaction shall not become a part of his/her personnel file for use against him/her at any time. The employee’s reply to such complaint, accusation or expression of dissatisfaction shall become part of his/her personnel file.

  • Compliance Report The Subservicer agrees that it shall permit, not more than once per year, the Servicer, the Issuer, the Indenture Trustee or the Indenture Administrator, as the Indenture Trustee’s designee, to conduct or have conducted a procedural audit regarding the Subservicer’s compliance with the requirements of the Higher Education Act or the terms of this Agreement. Such audits shall be at the expense of the Servicer.
