Health Coverage. For the duration of the leave required under this policy, not to exceed twelve (12) weeks, the Board will maintain the employee’s health coverage under any group health plan at the same level and under the same conditions as if the employee had continued to work. Any employee contributions to the health plan must be maintained during the leave to maintain coverage.

In addition, the Company shall pay to the Executive, in a lump sum on the Payment Date, an amount equal to the difference between (A) the Company’s reasonable determination of present value of the continuation of the benefits described in this Section 2(f) for 24 months and (B) the Company’s reasonable determination of the present value of the benefits the Executive will receive under Section 2(f)(ii) above.
Health Coverage. Executive’s medical and dental coverage shall cease upon the termination of Executive’s employment. In the event of such termination in accordance with the terms of this Section 4, Employer shall provide Executive with notice and enrollment materials confirming Executive’s right to continue medical and dental insurance coverage to the extent permitted under COBRA; provided, however, that Executive shall only be required to pay the premiums charged to similarly-situated active employees during the entire COBRA continuation period, and Employer shall pay the remaining cost of coverage.
