Base Salary from Executive Employment Agreement

This Executive Employment Agreement (the Agreement) is entered into between Audentes Therapeutics, Inc. (the Company) and (the Executive). This Agreement is effective as of , 20 (the Effective Date). [This Agreement supersedes and replaces in its entirety the dated , between Executive and the Company.]1

Base Salary. During the Employment Term, Executives annual base salary will be $ , payable in accordance with the Companys normal payroll practices, less any payroll deductions and withholdings as are required by law. Executives base salary in effect from time to time is referred to herein as the Base Salary.

Base Salary from Executive Employment Agreement

THIS AGREEMENT is made as of this 19th day of October, 2015, by and between NN, Inc., a Delaware Corporation with its principal place of business in Johnson City, Tennessee (the Company), and John A. Manzi (the Executive).

Base Salary. The Company shall pay to the Executive an aggregate base salary at a rate of Three Hundred Nineteen Thousand Dollars (USD $319,000.00) per annum, payable in accordance with the Companys normal payroll practices. Such base salary shall be subject to annual review by the Board of Directors of the Company and/or its Compensation Committee and may be changed from time to time in the discretion of the Board of Directors and/or the Compensation Committee or in accordance with the normal business practices of the Company. Effective as of February 1, 2017, the aggregate base salary in effect at such time shall be increased by $14,400. Annual Bonus. Beginning in fiscal year 2016, the Executive shall be given the opportunity to earn an annual incentive bonus for each fiscal year of the Company in accordance with the annual bonus plan and payment policies generally applicable to the Companys officers, as the same may be in effect from time to time. The Executives target annual incentive bonus opportunity shall be no less than 50% of his base salary for such year, but shall be dependent upon the achievement of the applicable performance goals established for such year. The Executive will be eligible to participate beginning 2016. For the year ending December 31, 2015, the Executive shall be paid a bonus with respect to the performance of the Companys subsidiary, Precision Engineered Products Holdings, Inc., in accordance with the terms and conditions of the Amended and Restated Precision Engineered Products LLC CEO, CFO and Presidents Annual Corporate Bonus Plan.

Base Salary from Amended and Restated Employment Agreement

THIS AMENDED AND RESTATED EMPLOYMENT AGREEMENT (this "Agreement") by and between LHC Group, Inc., a Delaware corporation (the "Company"), and Keith G. Myers ("Executive"), is dated as of April 1, 2017 (the "Agreement Date").

Base Salary. During the Term, the Company will pay to Executive base salary at the rate of Seven Hundred Thirty-Five Thousand Dollars ($735,000) per year ("Base Salary"), less normal withholdings, payable in approximately equal bi-weekly or other installments as are or become customary under the Company's payroll practices for its employees from time to time. The Compensation Committee of the Board of Directors of the Company (or the full Board, if there is no Compensation Committee) shall review Executive's Base Salary annually and may increase (but not decrease) Executive's Base Salary from year to year. Such adjusted salary then shall become Executive's Base Salary for purposes of this Agreement. The annual review of Executive's salary by the Board will consider, among other things, Executive's own performance, and the Company's performance.

Base Salary from Employment Agreement

This Employment Agreement (the "Agreement") is made as of May 2, 2016 (the "Effective Date"), between Patheon Pharmaceutical Services Inc. (the "Company") and Michel Lagarde (the "Executive").

Base Salary. The Company shall pay the Executive an annualized base salary ("Annual Base Salary") of U.S. $750,000, payable pursuant to the Company's regular payroll practices for its executives in effect at the time, subject to applicable withholdings and deductions. The Annual Base Salary shall be reviewed by the Chief Executive Officer and the Compensation Committee, for increase only, at such time as the salaries of other senior executives of the Company are reviewed generally.

Base Salary from Amended and Restated Employment Agreement

This Amended and Restated Employment Agreement (the "Agreement") is made as of October 19, 2015 (the "Effective Date"), between Patheon Pharmaceutical Services Inc. (the "Company") and Francisco Negron (the "Executive").

Base Salary. The Company shall pay the Executive an annualized base salary ("Annual Base Salary") of U.S. $425,000, retroactive to September 1, 2015 and payable pursuant to the Company's regular payroll practices for its executives in effect at the time, subject to applicable withholdings and deductions. The Annual Base Salary shall be reviewed by the Chief Executive Officer and the Compensation Committee, for increase only, at such time as the salaries of other senior executives of the Company are reviewed generally.

Base Salary from Executive Employment Agreement

This Executive Employment Agreement ("Agreement") is made and entered into on this 14th day of March, 2017 by and between KalVista Pharmaceuticals, Inc., a Delaware corporation (the "Company"), and Benjamin L. Palleiko (hereinafter, the "Executive").

Base Salary. The Executive shall receive a Base Salary at the annual rate of $340,000.00 during the Term of Employment, with such Base Salary payable in installments consistent with the Company's normal payroll schedule, subject to applicable withholding and other taxes. The Base Salary shall be reviewed, at least annually, for merit increases and may, by action and in the discretion of the Board (or its Compensation Committee), be increased at any time or from time to time, but may not be decreased from the then current Base Salary.

Base Salary from Executive Employment Agreement

This Executive Employment Agreement ("Agreement") is made and entered into on this 14th day of March, 2017 by and between KalVista Pharmaceuticals, Inc., a Delaware corporation (the "Company"), and T. Andrew Crockett (hereinafter, the "Executive").

Base Salary. The Executive shall receive a Base Salary at the annual rate of $450,000.00 during the Term of Employment, with such Base Salary payable in installments consistent with the Company's normal payroll schedule, subject to applicable withholding and other taxes. The Base Salary shall be reviewed, at least annually, for merit increases and may, by action and in the discretion of the Board (or its Compensation Committee), be increased at any time or from time to time, but may not be decreased from the then current Base Salary.

Base Salary from Employment Agreement

This Employment Agreement is made as of November 20, 2015, by Dominos Pizza LLC, a Michigan limited liability company (the Company) and Judith L. Werthauser (the Executive).

Base Salary. The Company shall pay the Executive a base salary at the rate of Four Hundred Fifty Thousand Dollars ($450,000.00) per year, payable in accordance with the payroll practices of the Company for its executives and subject to such increases as the Board of Directors of the Company or the Compensation Committee (the Board) in its sole discretion may determine from time to time (the Base Salary).

Base Salary from Employment Agreement

THIS EMPLOYMENT AGREEMENT (this "Agreement") is made this 13th day of October, 2016 (the "Signing Date"), to become effective the _1st__ day of November, 2016 (the "Effective Date"), by and between Allscripts Healthcare Solutions, Inc., a corporation organized and existing under the laws of the State of Delaware ("Company"), and Lisa Khorey ("Executive"). Provided, however, that the Effective Date may be changed by Executive to a later date of up to six months after Executive gives notice of her resignation from employment with current employer as may be required to comply with Executive's contractual obligations to her current employer.

Base Salary. During the Employment Period, Company shall pay to Executive an annual base salary at a rate of $430,000 per annum, subject to all appropriate federal and state withholding taxes, which base salary shall be payable in accordance with Company's normal payroll practices and procedures. Executive's base salary shall be reviewed annually during the Employment Period by the President and/or and the Chief Executive Officer of the Company, who shall recommend any increases to the Compensation Committee (the "Compensation Committee") of the Board of Directors of the Company (the "Board") and may be increased in the sole discretion of the Compensation Committee (organization) values">Board or Compensation Committee based on Executive's performance during the preceding calendar year. Executive's base salary, as such base salary may be increased hereunder, is hereinafter referred to as the "Base Salary."

Base Salary from Severance Agreement

THIS SEVERANCE AGREEMENT ("Agreement") is entered into as of January 3, 2017 ("Effective Date") between CarMax, Inc., a Virginia corporation, and its affiliated companies (collectively, the "Company"), and Edwin J. Hill (the "Executive").

Base Salary. During the Term, the Company shall pay the Executive a base salary ("Base Salary") in an amount established and approved by the Compensation and Personnel Committee of the Board ("Compensation Committee"); provided, however, that such Base Salary shall be established at a rate of not less than $600,000.00 per year, except as otherwise provided in this Section 5.1 below. This Base Salary shall be subject to all appropriate federal and state withholding taxes and payable in accordance with the normal payroll practices of the Company. The Compensation Committee shall review and adjust the Base Salary as it deems appropriate at least annually during the Term; provided, however, that the Executive's Base Salary shall not be decreased without the Executive's written consent, other than across-the-board reductions applicable to all senior officers of the Company. If adjusted, the Base Salary shall be so adjusted for all purposes of this Agreement.
