Supplier Performance Plan definition

Supplier Performance Plan means a formal quality plan for Supplier developed jointly by Supplier and Applied which complies with the requirements of Attachment 16.
Supplier Performance Plan means a formal performance plan for Supplier in the form of Attachment 16, as modified from time to time. Attachment 16 is the Supplier Performance Plan in effect on the Effective Date.

Performance Measures may vary from Performance Period to Performance Period and from Participant to Participant, and may be established on a stand-alone basis, in tandem or in the alternative.

  • Service program or “service” means any medical care ambulance service or nontransport service that has received authorization by the department.

  • Individual service plan means the written description of services, supports, and activities to be provided to an individual.
