Summary of Performance is a plan developed for a student with a disability at the secondary level whose eligibility for special education services terminates because of graduation with a regular diploma or reaching the maximum age of eligibility for special education services. The Plan must provide the student with a summary of the student’s academic achievement and functional performance, which shall include recommendations on how to assist the student in meeting his or her postsecondary goals.
