Successful Security Incident definition

Successful Security Incident means a Security Incident that results in the unauthorized Access, Use, Disclosure, modification, or destruction of information or interference with system operations in an Information System.
Successful Security Incident means any Security Incident (as defined in 45 CFR 164.304) that results in the unauthorized use, access, disclosure, modification or destruction of electronic Protected Health Information.
Successful Security Incident means a Security Incident that results in the unauthorized access, use, disclosure, modification, or destruction of PHI.

  • If the Successful Security Incident constitutes a Breach, the parties will proceed as required under this Agreement as to a Breach.

  • Successful Security Incidents will be reported to Covered Entity within two (2) business days of the date the Successful Security Incident is, or in the exercise of reasonable efforts should have been known, to Business Associate.

  • For Successful Security Incidents, Doctor shall give notice to CCMI not more than five (5) days after Doctor learns of the Successful Security Incident.

  • Business Associate shall report to Covered Entity any of the following events within two (2) business days of becoming aware of the occurrence of the event: Any Use or Disclosure of PHI not authorized by this Addendum; Any Successful Security Incident; and Any acquisition, access, Use or Disclosure of Unsecured PHI in a manner not permitted by the HIPAA Privacy Rule.

  • Subcontractor must report to the Attorney General any Use or Disclosure of PHI not provided for in this Agreement, any Successful Security Incident involving ePHI, and any Breach of Unsecured PHI as required by 45 C.F.R. §164.410.


More Definitions of Successful Security Incident

Successful Security Incident means a Security Incident that results in the unauthorized access, Use, Disclosure, modification, encryption, or destruction of Subcontractor’s PHI.
Successful Security Incident means any Security Incident (as defined in 45 CFR 164.304) that results in the unauthorized use, access, disclosure, modification or destruction of electronic Protected Health Information. All capitalized terms used in this Agreement and not defined elsewhere herein or in the Services Agreement shall have the same meaning as those terms as used or defined in the HIPAA Rules.
Successful Security Incident means successful unauthorized access, use, disclosure, modification, or destruction of Electronic PHI or interference with system operations in an Information System in a manner that materially risks the Confidentiality, Integrity, or Availability of such PHI. Notice is hereby deemed provided, and no further notice will be provided, for unsuccessful attempts at such unauthorized access, use, disclosure, modification, or destruction, such as pings and other broadcast attacks on a firewall, denial of service attacks, port scans, unsuccessful login attempts, or interception of encrypted information where the key is not compromised, or any combination of the above.
Successful Security Incident means a Security Incident that results in the unauthorized Access, Use,Disclosure, modification, or destruction of information or interference with system operations in an Information System. “Unsuccessful Security Incident” shall mean a Security Incident such as routine occurrences that do not result in unauthorized Access, Use, Disclosure, modification, or destruction of information or interference with system operations in an Information System, such as: (i) unsuccessful attempts to penetrate computer networks or services maintained by Business Associate; and (ii) immaterial incidents such as pings and other broadcast attacks on Business Associate's firewall, port scans, unsuccessful log-on attempts, denials of service and any combination of the above with respect to Business Associate’s Information System.

Successful Security Incident means accidental, unauthorized or unlawful access to, destruction of, loss of, alteration of or unauthorized disclosure of Customer Data while being Processed by AAI. In the event of a Successful Security Incident, AAI shall notify the Customer without undue delay after becoming aware of it, but in no event longer than 72 hours. Such notification shall detail the following, if known; (i) the nature of the unauthorized use or disclosure; (ii) who made the unauthorized use or received the unauthorized disclosure; (iii) the steps AAI has taken to mitigate any deleterious effect of the unauthorized use or disclosure;
