Substance Abuse Care means medically necessary care provided by an eligible provider for the illness or condition that OptumHealth has determined:

  • If Mental Health Services and/or Substance Abuse Care are not preauthorized, the benefits will be reduced as shown in the Schedule Page.Orthotic and Prosthetic Devices – Coverage is provided for Orthotic and Prosthetic Devices, other than a dental or cranial prosthetic, which meets minimum specifications for the body part it is replacing regardless of the functional activity level.
