Specialist Provider definition

Specialist Provider means an allopathic or osteopathic physician or ARNP, whose practice is limited to a particular specialty and is either board certified or board eligible and who meets FCHN’s credentialing standards, and under the terms of this Agreement, agrees to provide Covered Services to Participants within the scope of his/her license.
