Benefits are available for treatment in the Skilled Nursing unit of a Hospital or in a free- standing Skilled Nursing Facility (SNF) when you are receiving Skilled Nursing or rehabilitative services.
Subsequent to an inpatient hospital stay, this plan covers the Medicare Part A copayment for Medicare eligible expenses for a SNF from the 21st day through the 100th day in a Medicare benefit period.
These include Medicare SNF days and Medicaid room and board days provided in swing beds that meet all other requirements for use of swing beds, including claims processing procedures and Minnesota Department of Health approval.
The benefit period ends when you haven’t received any inpatient hospital care (or skilled care in a SNF) for 60 days in a row, including your day of discharge.
All MCO enrollees will remain continuously enrolled throughout the term of this Contract, except in situations where clients lose their Medicaid eligibility, are admitted to a skilled nursing facility (SNF) or nursing facility, voluntarily disenroll, or are re-categorized into a Medicaid coverage category not included in the managed care delivery system.
