Service Delivery Location definition

Service Delivery Location. (SDL) means a public health office or a Community Health Centre
Service Delivery Location is defined in SS Section 1.1; “Service Provider” is defined in the Form of Agreement;
Service Delivery Location means the office, facility, location, or other physical premises where the applicant or provider intends to provide or currently provides behavioral health treatment services.
