  • If, following termination of this Agreement, Provider's facility(ies) is (are) closed or if majority ownership of Provider changes, then within forty-eight hours thereafter, Director of SDHCS and County Director shall be notified thereof by Provider in writing of all arrangements made by Provider for preservation of all the patient/client, financial, and other records referred to in this Paragraph 12.

  • If Title XIX Short-Doyle/Medi-Cal Specialty Mental Health Services are provided under this Agreement, Provider authorizes County to serve as the fiscal intermediary for claiming and reimbursement for such Title XIX Short-Doyle/Medi-Cal Specialty Mental Health Services to act on Provider's behalf with SDHCS and/or SDSS in regard to claiming reimbursement for Title XIX Short-Doyle/Medi-Cal Specialty Mental Health Services.

  • Provider agrees that for a period of seven years or until final audit is completed, whichever occurs later, following the furnishing of services under this Agreement, Provider shall maintain and make available to the State Department of Health Care Services (SDHCS) and any other authorized state agencies, or to any of their duly authorized representatives, the contracts, books, documents and records of Provider which are necessary to verify the nature and extent of the cost of services hereunder.

  • County is making these provisional payments to Provider prior to receiving FFP reimbursement for payment or notification from SDHCS to County of approved specialty mental health services claimed by the County to the State on behalf of the Provider.

  • All staff providing services under this Agreement shall be qualified and shall possess all appropriate licenses in accordance with W&I Section 5603 and all other applicable requirements of the California Business and Professions Code, W&I, CCR, CR/DC Manual, SDHCS Policy Letters, and function within the scope of practice as dictated by licensing boards/bodies.

  • Provider shall be solely liable and responsible for all data and information submitted by Provider to County in support of all claims for Title XIX Short-Doyle/Medi-Cal Specialty Mental Health Services submitted by County as the fiscal intermediary to, SDHCS and/or SDSS and for any subsequent State approvals or denials of such claims that may be based on data and information submitted by Provider.

  • Provider shall certify in writing that all necessary documentation exists prior to submission of the Short-Doyle Medi-Cal claim to SDHCS for the Title XIX Short- Doyle/Medi-Cal Specialty Mental Health Services.

  • All staff providing services under this Agreement shall be qualified and shall possess all appropriate licenses in accordance with W&I Section 5751.2 and all other applicable requirements of the California Business and Professions Code, W&I, CCR, CR/DC Manual, SDHCS Policy Letters, and function within the scope of practice as dictated by licensing boards/bodies.

  • All staff providing services under this Agreement shall be qualified and shall possess all appropriate licenses in accordance with WIC Section 5603 and all other applicable requirements of the California Business and Professions Code, WIC, CCR, CR/DC Manual, SDHCS Policy Letters, and function within the scope of practice as dictated by licensing boards/bodies.

  • aepaccmeצc means the department of public health.

  • DSS means the Department of Social Services.

  • SFIs means Standing Financial Instructions.

  • ACS means “asset-controlling supplier” as that term is defined in the Cap and Trade Regulations.

  • ATS means an alternative trading system, as defined in Rule 300(a)(1) of Regulation ATS under the Exchange Act.

  • SDS means safety data sheets;

  • IEC means specifications of International Electro-technical Commission;

  • FIS means free into store delivery, which requires the Supplier to pay all costs and be responsible for all requirements associated with the supply and delivery of Goods to the Company at the delivery site described in the Order, including all costs associated with transport, insurances, licences, authorisations, duties and taxes.

  • Comcast means Comcast Corporation, a Pennsylvania corporation.

  • SCS means the Soil Conservation Service.

  • Network pharmacy means any pharmacy that has an agreement to accept our pharmacy allowance for prescription drugs and diabetic equipment/supplies covered under this agreement. All other pharmacies are NON-NETWORK PHARMACIES. The one exception and for the purpose of specialty Prescription Drugs, only specialty pharmacies that have an agreement to accept our pharmacy allowance are network pharmacies and all others pharmacies are non-network pharmacies.

  • DHS means the Department of Human Services.

  • FMCSA means Federal Motor Carrier Safety Administration.

  • CMC means Comprehensive maintenance Contract (labour, spare and preventive maintenance)

  • Integrated Services Digital Network (ISDN means a switched network service that provides end-to-end digital connectivity for the simultaneous transmission of voice and data. Basic Rate Interface-ISDN (BRI-ISDN) provides for a digital transmission of two (2) 64 Kbps bearer channels and one (1) 16 Kbps data channel (2B+D).

  • DTS means Days to Settlement, i.e., the number of actual days elapsed from and including the original Closing Day with respect to such Accepted Note (in the case of the first such payment with respect to such Accepted Note) or from and including the date of the next preceding payment (in the case of any subsequent delayed delivery fee payment with respect to such Accepted Note) to but excluding the date of such payment; and "PA" means Principal Amount, i.e., the principal amount of the Accepted Note for which such calculation is being made. In no case shall the Delayed Delivery Fee be less than zero. Nothing contained herein shall obligate any Purchaser to purchase any Accepted Note on any day other than the Closing Day for such Accepted Note, as the same may be rescheduled from time to time in compliance with paragraph 2B(7).
