Release Form definition

Release Form means a declaration and publicity release form.
Release Form means a release agreement which is to be signed by the Eligible Employee releasing any and all claims against the Employer and which is in such form as approved by the Company.
Release Form means a document provided by the hospital to the patient at the time of the sexual assault examination that gives the patient the option of authorizing, in writing, the release of the kit to law enforcement.
