Qualified medical child support order”or “QMCSO means a medical child support order that creates or recognizes the existence of an alternate recipient’s right to, or assigns to an alternate recipient the right to, receive health benefits for which a participant or eligible dependent is entitled under this Plan. In order for such order to be a qualified medical child support order, it must clearly specify the following:• The name and last known mailing address (if any) of a participant and the name and mailing address of each such alternate recipient covered by the order;• A reasonable description of the type of coverage to be provided by the Plan to each alternate recipient, or the manner in which such type of coverage is to be determined; • The period of coverage to which the order pertains; and • The name of this Plan. In addition, a national medical support notice shall be deemed a qualified medical child support order if it: • Contains the information set forth above in the definition of national medical support notice; • Identified either the specific type of coverage or all available group health coverage. If the participating employer received a national medical support notice that does not designate either specific types of coverage or all available coverage, the participating employer and the Plan Administrator will assume that all are designated; • Informs the Plan Administrator that, if a group health plan has multiple options and a participant is not enrolled, the issuing agency will select the Core Plan after the national medical support notice is qualified; and • Specifies that the period of coverage may end for the alternate recipient(s) only when similarly situated dependents are no longer eligible for coverage under the terms of the Plan or upon the occurrence of certain specified events. However, such an order need not be recognized as “qualified” if it requires the Plan to provide any type or form of benefit, or any option, not otherwise provided to a participant and eligible dependents, except to the extent necessary to meet the requirements of a state law relating to medical child support order, as described in Social Security Act § 1098 (as added by the Omnibus Budget Reconciliation Act of 1993 §13822).

  • Qualified Medical Child Support Order means a Medical Child Support Order which creates (including assignment of rights) or recognizes an Alternate Recipient's right to receive benefits to which a Participant or Qualified Beneficiary is eligible under this Plan, and has been determined by the Plan Administrator to meet the qualification requirements as outlined under “Procedures” of this provision.

  • Medical Child Support Order means any judgment, decree or order (including approval of a domestic relations settlement agreement) issued by a court of competent jurisdiction that:

  • Qualified medical provider means the same as that term is defined in Section 26-61a-102.

  • Child support order means a support order for a child, including a child who has attained the age of majority under the law of the issuing state or foreign country.

  • Qualified Medical Practitioner means any person legally authorized by the Government with jurisdiction in the geographical area of his or her practice to render medical or surgical service, but excluding a qualified medical practitioner who is the Insured Person or an Immediate Family Member of the Insured Person.

  • Medical cannabis card means the same as that term is defined in Section 26-61a-102.

  • the Scottish Infected Blood Support Scheme means the scheme of that name administered by the Common Services Agency (constituted under section 10 of the National Health Service (Scotland) Act 1978(b));

  • Authorized medical physicist means an individual who:

  • National Medical Support Notice or “NMSN” shall mean a notice that contains the following information:

  • Non-Participating Certified Clinical Nurse Specialist means a Certified Clinical Nurse Specialist who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Participating Durable Medical Equipment Provider means a Durable Medical Equipment Provider who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Approved abuse education training program means a training program using a curriculum approved by the abuse education review panel of the department of public health or a training program offered by a hospital, a professional organization for physicians, or the department of human services, the department of education, an area education agency, a school district, the Iowa law enforcement academy, an Iowa college or university, or a similar state agency.

  • Child support means that part of a Child Restraint System by which the child can be raised within the Child Restraint System.

  • QMCSO means a qualified medical child support order, as defined in ERISA § 609(a).

  • Peer-reviewed medical literature means a scientific study published only after having been critically

  • Adult foster care facility means an adult foster care facility licensed under the adult foster care facility licensing act, 1979 PA 218, MCL 400.701 to 400.737.

  • Participating Certified Nurse Practitioner means a Certified Nurse Practitioner who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Non-Participating Durable Medical Equipment Provider means a Durable Medical Equipment Provider who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Child support services means any civil, criminal or administrative action taken by the Division of

  • Spousal support order means a support order for a spouse or former spouse of the obligor.

  • Medical cannabis means the same as that term is defined in Section 26-61a-102.

  • Non-Participating Certified Nurse-Midwife means a Certified Nurse-Midwife who does not have a written agreement with Blue Cross and Blue Shield of Illinois or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Child support agency means the public agency responsible for child support

  • Support order means a judgment, decree, order, decision, or directive, whether temporary, final, or subject to modification, issued in a state or foreign country for the benefit of a child, a spouse, or a former spouse, which provides for monetary support, health care, arrearages, retroactive support, or reimbursement for financial assistance provided to an individual obligee in place of child support. The term may include related costs and fees, interest, income withholding, automatic adjustment, reasonable attorney's fees, and other relief.

  • Non-Participating Certified Nurse Practitioner means a Certified Nurse Practitioner who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Participating Certified Clinical Nurse Specialist means a Certified Clinical Nurse Specialist who has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.
