Protected Health Information (PHI) definition

Protected Health Information (PHI) means individually identifiable health information created, received, maintained or transmitted by Business Associate on behalf of a health care component of the Covered Entity that relates to the provision of health care to an Individual; the past, present, or future physical or mental health or condition of an Individual; or the past, present, or future payment for provision of health care to an Individual. 45 CFR 160.103. PHI includes demographic information that identifies the Individual or about which there is reasonable basis to believe can be used to identify the Individual. 45 CFR 160.103. Protected Health Information (PHI) collectively means protected health information as defined in Code of Federal Regulations Title 45, Subtitle A, Subchapter C, Part 160, Section 160.103 and medical information as defined in California Civil Code Section 56.05.
